Full-time (ALL) shifts available 7am-3pm, 3pm-11pm, 11pm-7am

Starting at $37.00 - $46.25

Salary is based on years of experience!

Sign-on Bonus of $2000 - 3rd shift only.

Essential Functions

  • Practices nursing within the scope of education and ability.

  • Supports Presbyterian Homes’ policies and procedures and Nursing Department standards of care.

  • Observes and preserves residents’ rights.

  • Communicates appropriately with supervisors, co-workers, other employees, residents and visitors.

  • Accepts accountability for clinical care of assigned residents.

  • Makes rounds on residents at least three times each shift assessing for changes in condition and response to care and treatment.

  • Monitors the status of residents with acute problems.

  • Provides direct nursing care to residents who require professional skilled care.

  • Administers medications and treatments in accordance with established policies and procedures. Is familiar with indications of use, action, and major side effects of medications administered.

  • Takes direct appropriate action in urgent and emergent situations.

  • Anticipates and acts to maintain a safe environment.

  • Supervises nursing assistant staff in direct resident care by:

  • Giving verbal and written report at the beginning of each shift.

  • Observing CNA / resident interaction and completion of resident care including bathing, grooming, dressing, feeding, toileting, mobility and attendance at activities and appointments.

  • Discussing a plan of care for individual resident.

  • Making rounds and receiving feedback on resident status at the end of the shift or at any time there is a change in a resident’s condition.

  • Communicating information regarding job performance on an ongoing basis.

  • Evaluates, in collaboration with the Director of Nursing or Supervisor, the job performance of CNAs.

  • Completes a nursing assessment upon admission, considering physical, functional and psychosocial status.

  • Develops and keeps current nursing care plans of all residents on assigned unit ensuring that condition changes are duly noted.

  • Knowledgeable of the assessments and evaluations of other disciplines and incorporates them into resident care.

  • Communicates changes in condition to supervisor and to patient families as assigned.

  • Participates in interdisciplinary care conferences and collaborates with other disciplines as needed.

  • Communicates with physicians regarding the residents’ status.

  • Maintains a therapeutic relationship with residents.

  • Documents appropriate and necessary information in the medical record.

  • Exchanges pertinent information in the inter-shift report.

  • Incorporates knowledge of normal aging changes into nursing practice.

  • Is knowledgeable of common health problems of older adults and their usual treatment.

  • Participates in staff development programs and unit meetings.

  • Assists in the orientation of new staff.

  • Demonstrates leadership skills.

  • Seeks supervision and guidance as needed.

  • Channels administrative problems appropriately to Nurse Manager / Supervisor.

  • Daily use of all computer systems is provided by Presbyterian Homes in support of care and service to residents/patients, and to support administrative responsibilities. Attend all facility software classes to maintain computer skills. Demonstrates skills to staff and leads by encouraging others to use computer resources.

Skills/Qualifications

  • Graduation from an approved registered professional nursing program.

  • MUST BE Licensed through the state of Illinois.

  • One year as an RN in a hospital (general care) or long-term care setting with adults preferred.

  • Good communication skills. Interest in working with older adults. Ability to direct nursing assistants. Knowledge of aging changes and common health problems of geriatric patients.

  • CPR certification.

  • Familiarity and competence in basic computer operations, email, word processing and scheduling.
