JOB REQUIREMENTS: Nurse - LPN or RN Eagle Crest Communities is seeking Nurses to join the team of healthcare professionals at our skilled nursing and rehab facility, Riverside Transitional Care. These individuals will bring their high-quality clinical skills and can-do attitude to the team. The successful candidates can be either a Licensed Practical Nurse or Registered Nurse. What You\'ll Do: -Responsible for providing assistance to our rehab, geriatric, and or/memory care populations -Coordinate, implement and evaluate nursing care -Provide treatments -Administer medications -Assist with activities of daily living as needed -Provide guidance and leadership when necessary to Nurse Technicians, Certified Nursing Assistants, Certified Medication Assistants, and Support Aids. What You\'ll Need: -Current license to practice in the state of Wisconsin as a Licensed Practical Nurse or Registered Nurse. -Current CPR Certification -An expert in any chronic geriatric disease is a plus, but we will train the right fit for our team! -Previous experience working in long term care or previous acute care experience, preferred Other Responsibilities of Job: Record patients\' medical information and vital signs. Administer medications to patients and monitor patients for reactions or side effects. Maintain accurate, detailed reports and records. Monitor, record, and report symptoms or changes in patients\' conditions. Provide health care, first aid, immunizations, or assistance in convalescence or rehabilitation in locations such as schools, hospitals, or industry. Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans. Direct or supervise less-skilled nursing or healthcare personnel or supervise a particular unit. Monitor all aspects of patient care, including diet and physical activity. Modify patient treatment plans as indicated by patients\' responses and conditions. Observe nurses and visit patients to ensure proper nursing care. Assess the needs of individuals, families, or communities, including assessment of individuals\' home or work environments, to identify potential health or safety problems.
