We also offer psychiatric inpatient care, and we deliver therapeutic services through our community outreach clinics. We are a leader in the Trauma-Informed Care Movement, and provide training, coaching and consulting to outside organizations.

Job Description

The Cultural Community Health Worker (CCHW) serves as an active promoter of health for the Black, Indigenous, and People of Color (BIPOC) communities. The CCHW provides culturally appropriate, outreach, and health education; addresses social determinants of health, provides information regarding access to needed services, and assists individuals in getting the care they need. The CCHW advocates for individual and community health and social needs, and provides some direct services such as helping people complete applications to access health benefits; and serves community residents where they live, eat, work, and play. The CCHW is a frontline agent of change, and seeks to promote human rights and social justice by working and advocating to reduce health disparities in underserved communities.

  • Screens patients for social determinant of health needs at regular primary care, specialty care, emergency department visits and in the community.

  • Assists patients with public benefit enrollment, including health insurance, Medicaid and Charitable Care.

  • Accompanies patients to appointments, travels to patient homes, community locations, various agencies, events, and other outreach destinations as needed.

  • Communicates with patients on a daily basis using a variety of mediums (phone, email, video remote, and in-person) to screen for SDOH, refer to services, follow-up and answer questions.

  • Creates an inclusive and collaborative environment for providers and patients alike to help break down bias, prejudice, and other institutional barriers.

  • Builds relationships with community resource partners. Creates and maintains community resource inventory and identifies existing health and public health local immigrant and refugee-focused resources available to the target refugee and immigrant populations within KCMO.

  • Facilitates communication for Limited English Proficient (LEP) patients; Serves as an advocate and cultural mediator as necessary and when appropriate to enhance patient centered care; Ensures patient’s access to interpreters and translators for medical and social service appointments.

  • Accurately tracks and monitors data to assure timely patient follow-up. Tracks services provided and number of patients linked to care. Generates qualitative and quantitative reports as necessary, for program monitoring and evaluation.

  • As directed provides assistance in the operation of the One World Pantry. This can include, but not limited to, driving to pick up products, unloading and storing food donations and deliveries, ensuring food pantry, office space, storage room, etc. are all clean and well organized and interacting with guests to the department, assessing for nutrition needs, healthcare needs, resource needs and available financial support for healthcare.

Minimum Requirements

  • High School diploma/GED.

  • One year of CHW experience or comparable experience.

  • Comfortable conducting home visits.

  • Strong interpersonal skills demonstrative of empathy and support.

  • Demonstrate ability to work collaboratively.

  • Demonstrate ability to work with families who face exceptional challenges.

  • Willingness to visit with families in hospital and the community

  • Bilingual English/Spanish preferred.

  • Proficiency in Microsoft Office.

  • If driving is a requirement for the employee, he/she must meet the requirements specified in the Fleet Driving Safety and Compliance Policy and must continue to meet these requirements as a condition of employment.
