The person in this position will serve as a liaison between communities and health care agencies.  Responsible for conducting assessments to identify community members'  health needs and develop individualized plans to address those needs, conduct outreach efforts to engage with community members and promote community health initiatives, assist community members in navigating the healthcare system and accessing necessary resources, provide case management and follow up to ensure that the linkages to services and supports addressed the participant's needs,  Qualifications for this position include project coordination and organizational skills, ability to manage multiple projects with attention to detail, meet deadlines, ability to handle interruptions and produce timely, accurate work, previous experience in a community health or related role.  Bachelor's degree in related field preferred or commensurate experience, previous experience in the human services and social work preferred.  This is a full time position with benefits to include health and vision coverage, paid sick, vacation, personal and holiday time.  EOE.
