UNC Health Care Lead Social Worker in Hendersonville, North Carolina

Summary:

The Lead Social Worker provides direct service functions consistent with a staff social worker as well as administrative duties that support the unit, the organization, and social work colleagues. Monitors social work service provision to ensure quality care and service delivery, evaluates and supervises social work staff, trains new social workers in the department and serves as a resource person. Works with community service providers to develop protocols for smooth transition of care and participates in community meetings that address the continuum of care available for patients.

Responsibilities:

• Completes a biopsychosocial assessment of patients; assists patients to develop a discharge plan; obtains collateral information from family members, community providers and other supports as appropriate; provides crisis intervention and counseling as needed to facilitate a safe plan of care; serves as a patient advocate, representing patient’s interest to hospital team and obtains, interprets, and communicates to staff and physicians necessary information regarding patients.

• Assists inpatients and families prepare for discharge through coordination of care, referrals to appropriate services, education and consultation regarding resources, and placement as needed. Maintains communication with patent’s family members and/or significant relationships as indicated throughout the course of hospitalization to facilitate discharge. Serves as patient advocate to secure adequate community services support and/or appropriate continuing care.

• Develops and maintains good working relationships with outside community providers and social agencies, collaborating as necessary regarding discharge plans. Reaches out to the community to learn of and utilize new resources for meeting patient needs. Assists patients and families with obtaining community assistance by referral to proper resources. Works with community providers to develop protocols for smooth transition of care and participates in community meetings that address the continuum of care available for patients.

• Provides regular and consistent participation in assigned hospital committees and duties, representing the profession of social work as well as the psychiatric unit.

• Provides supervision, monitoring and evaluation of social work staff and social work service delivery including training and orientation of new staff, developing policies and procedures for social work service and serving as a resource person for the department to address questions, solve problems, and ensure the smooth operation of the department.

Qualifications

Required

• Masters of Social Work degree

• Licensed and certified as a Licensed Clinical Social Worker (LCSW) in the state of North Carolina

• Experience performing individual, group, and family counseling

• Experience performing discharge planning

Preferred

• Two (2) years experience as a clinical social worker in a healthcare setting
