Position Purpose

Saint Francis Hospital and Medical Center is looking for a Manager of Social Work to join our Case Management Team.

The Manager of Social Work is responsible for assisting the Director of Case Management with the case management program, including the functions of social work, utilization management, and discharge planning for the Hospital.

You Will Be Responsible For:

  • Responsible for developing an infrastructure for assessing, determining, and coordinating the discharge needs of patients to facilitate optimum outcomes and throughput.

  • staffing levels, competency and educational needs of the case management staff and effectively coordinates and controls resources.

  • participates in individual interventions to assist the psychiatric patient towards recovery and rehabilitation.

  • Oversees the Social Worker staff

Minimum Requirements:

  • Education: master’s degree in social work required.

  • License: License Clinical Social Worker (LCSW) from State of CT required

  • Three years case management experience in a hospital setting.

  • Two years supervisory experience

  • Ability to effectively interact with all levels of hospital, governmental and medical staff.
