Trinity Health Complex Care Coordinator - Registered Nurse in Hartford, Connecticut

Position Purpose:

In the Emergency Department ED at Saint Francis Hospital and Medical Center , the Registered Nurse RN serving in the role of Complex Care Coordinator will specialize in managing the care of patients with complex medical, social, and/or behavioral health needs.

What you will do:

  • Conducting comprehensive assessments of patients’ medical, social, and behavioral health needs upon arrival in the Emergency Department ED.

  • Collaborating with the ER healthcare team to develop individualized care plans for complex patients.

  • Coordinator referrals to specialists, community resources, and support services on patients’ needs and preferences.

Minimum Qualifications :

  • Education : Completion of an accredited Nursing Program with a Bachelor’s degree in Nursing is mandatory. Possessing a Master’s degree in Nursing or another Healthcare field is preferred.

  • Registered Nurse RN Licensure: Must hold a valid and current Connecticut registered nurse licensure.

  • Experience: A minimum of 5 years of nursing experience in an Acute InPatient setting and/or Emergency Room nursing is required. Preference will be given to candidates with experience in Emergency Room nursing or Emergency Room Case Management.

  • BLS or CPR Certification: Current certification from the American Heart Association or American Red Cross is required.

Position Highlights and Benefits :

Full-Time: 40 hours per week on the Day Shift.

  • Variable shifts: five 8-hour shifts per week or four 10-hour shifts per week

  • Rotating weekends, flexible
