Trinity Health Community Health Worker - Breast Health Outreach in Hartford, Connecticut

Come join our Community Outreach team at Saint Francis Hospital, a member of Trinity Health Of New England!

Position Purpose

This Community Health Worker will be conducting and tracking community outreach efforts for The Comprehensive Women’s Health Center’s Breast Health Outreach Education Program. This includes coordinating and scheduling breast health outreach events in the community, facilitating and following up on appointments for breast cancer screening and diagnosis. The Community Health Worker will support the Women’s Health department of surgery in supporting patients identified by their provider as high risk and in need of social care support which can include: follow up appointments, connecting them to community resources as needed and goal setting to improve overall health and wellbeing.

What you will do

  • Coordinate and schedule breast health outreach in the Greater Hartford Community; facilitate appointments for breast cancer screening and diagnosis; attend health events at churches and other community organizations

  • Work during outreach/education sessions to encourage clients to make screening appointments and help facilitate those appointments

  • Deliver community education and resources to low-income, uninsured or under insured patients on importance of breast screening

  • Support patients undergoing planned surgery to ensure they are keeping their pre and post op appointments, conduct social needs screenings and developing a plan to address those barriers that may prevent patient from undergoing surgery or are at risk for readmission

Minimum Qualifications

  • Minimum high school diploma or equivalent, college preferred

  • Verifiable good driving record and reliable transportation

  • Personal experience residing or serving in underserved communities; experience in a community outreach education programs preferred but will provide on the job training and education

  • Willingness to be a strong advocate for patients in need

  • Ability to work independently with guidance from clinical supervisor. Ability to relate and connect with community in and around Hartford area
