Job Summary
Responsibility for coordinating the services provided by multiple disciplines within and outside the health system. Exercises independent judgment in the assessment, diagnosis initiation of selected protocols/procedures. Provides educational support for patients/significant others, health team members and the community and coordinates evaluation practices to measure outcomes of programs.
Essential Functions
Coordinates program planning, implementation, and evaluation in area of accountability in collaboration with members of the health care team. Identifies opportunities for philanthropic support to support program objectives. Acts as patient advocate, crossing multidisciplinary lines to meet patient needs. Communicates, collaborates, networks with and acts as a consultant to other members of the health care team within and outside Spectrum Health. Acts as a consult in area of expertise.
Assesses the patient/significant other, health care team, and community educational needs and develops, implements, evaluates and revises teaching strategies/educational programs related to area of specialty.
Develop and maintain network with internal and external partners to support programming. Develops support systems for patient population and coordinates program with other system-wide/community support programs. Participates in Hospital/Community committees as member and consultant on quality care issues.
Initiates performance improvement studies in order to evaluate and measure the outcome of services. Make recommendations to improve the quality of services provided.
Coordinates the delivery of patient care within Program services related to area of specialty.
Performs and provides designated activities related to area of specialty.
Maintains the documentation and record organizational and external agency requirements for services provided.
Oversees selected staff as assigned by manager to coordinate program services.
Qualifications
Required Bachelor's Degree or equivalent related field
Preferred Master's Degree or equivalent related field
2 years of relevant experience related experience Required
LIC-Registered Nurse (RN) - STATE_MI State of Michigan Excludes CV Administration, HM Patient Care Logistics, Health Comm - Department, GM-Community Education, Lifestyle Medicine, PD HDVCH Child Protection, PH Medical Affairs Admin, PH Pre-Service Utility Management required Or
LIC-License Practical Nursing (LPN) - STATE_MI State of Michigan required
CRT-Physician Asst Certified (PA-C) - NCCPA National Commission on Certification of Physician Assistants required Or
CRT-Basic Life Support (BLS) - AHA American Heart Association Excludes Trauma Dept. required
Ideal candidate will be an RN, LPN, NP, or PA possessing prior Cardiology experience and strong passion for Cardiology Patient Population
Ideal candidate will possess Accreditation and/or Quality Improvement experience
Ideal candidate will possess strong Critical Thinking skills, Solid Communication Skills, will be very Self-Motivated; someone who will be able independently identify and pursue process improvement opportunities, someone possessing genuine curiosity and desire to learn
This is a Monday to Friday position
