Metro Health Diabetes Educator in Grand Rapids, Michigan

Metro Health (now the University of Michigan Health-West) is looking for Diabetes Educator - Diabetes Education * Shift Varies - 28hrs/wk.

Under the direction of the Manager of Outpatient Diabetes Education, the responsibility of the Diabetes Educator is to provide education on diabetes self management to newly diagnosed or poorly controlled diabetes inpatients. Will be expected to work well in collaboration with team members and be involved in helping identify clinical problems. Collaborates with the Program Coordinator to plan, develop, promote and evaluate the program on an ongoing basis.

Requirements:

Required Licenses and Certification:

  1. Registered Dietitian OR Registered Nurse License in the State of Michigan Required
  2. Certified Diabetes Educator (CDE) preferred
  3. Current Basic Life Support (BLS/CPR) provider preferred

Minimum Education:

  1. Registered Dietitian or Associate's degree in Nursing required (Bachelor of Science in Nursing preferred)

Minimum Work Experience:

  1. Two years experience working with diabetes patients in an inpatient or outpatient arena.
  2. Patient teaching experience.

Skills/Abilities:

  1. Excellent communication and interpersonal skills required.
  2. Strong team skills needed.
  3. Able to work independently.
  4. Excellent time management and organizational skills.
  5. Able to work flexible hours to meet program needs.

Essential Functions and Responsibilities:

  • Assesses, plans, implements, collaborates, and evaluates patient/family/significant other for knowledge and learning needs, physical and psycho-social needs.
  • Documents and communicates pertinent findings and or problems/concerns to all team members.
  • Assists in developing and reviewing program goals, objectives and curriculum.
  • Assures statistics and billing are done accurately and in timely manner.
  • Participates in quality monitoring and process outcomes for the department.
  • Demonstrates knowledge of and adheres to standards of the American Association of Diabetes Educators and Michigan Department of Community Health and documentation for recertification is current and maintained.
  • Continues professional growth and maintains all needed CE's for license and credentials.
  • Adheres to standards of practice, policies and procedures and promotes safety within the organization
  • Inpatient educator to monitor patients and readmission rate within the Hospital setting
  • Participate in staff development and education of newest diabetes standards and be available as resource to the staff at the hospital.
