Nurse Practitioner NP Hospice

Purpose :

The Hospice Nurse Practitioner serves as a primary level care provider under the clinical supervision of the Hospice Medical Director. The NP (Nurse Practitioner) will make face-to-face assessments of hospice patients in a variety of settings under all levels of hospice care. The NP will collaborate with the members of the IDT (Interdisciplinary Team) to assess client eligibility for hospice and provide interventions as required for pain and symptom management. The NP demonstrates expertise in patient care. The NP may serve as a liaison with other hospice referral sources, providing consultation when necessary. The NP will maintain high standards of integrity and business ethics and provide all patient care in accordance with current professional standards of practice and regulatory requirements

What you will do:

  • Makes visits to clients in their place of residence and/or inhospital setting

  • Consistently and accurately assesses clients at admission, at required time points, and as needed, utilizing appropriate standardized documentation. Assesses the physical and psychosocial status of clients through interview, health history review, physical examination, and, when appropriate, diagnostic studies.

  • Evaluates clients for ongoing eligibility for hospice services by reviewing IDT (Interdisciplinary Team) plan of care, documentation and diagnostic data, and through collaboration with the Medical Director and client’s Attending Physician.

  • Informs the client/caregiver about the agency, care/services available, client obligation for payment, and other responsibilities and rights, before care is initiated. Accurately explains/understands the Hospice Benefit.

  • Participates in making decisions regarding admissions to Hospice home care or inpatient setting(s) in conjunction with the Hospice Interdisciplinary Team

  • Complies to specific laws and regulations with respect to Collaborative Agreement with Medical Director.

  • Order, perform, and interpret laboratory and diagnostic tests within the scope of professional practice and Hospice Guidelines.

  • Makes appropriate referrals for evaluation/care to other disciplines and services, and coordinates care with others to ensure effective and efficient care is provided.

Qualifications:

  • Graduate of an approved Nursing education program

  • Master’s Degree in nursing

  • Certified/licensed as a Nurse Practitioner in state in which practicing

  • DEA License in State in which practicing, if required

  • Must have full (non-restricted) primary active medical staff membership at local Trinity Hospital, and remain in good standing with appropriate clinical privileges

  • In good standing/able to participate in the Medicare & Medicaid programs

  • Minimum of 5 years nursing experience; clinical nursing care in oncology, hospice, geriatrics, intensive care preferred
