Trinity Health Pharmacist Resident in Ft. Lauderdale, Florida

Description:

Monitors Daily: IV to PO's, renal dosage adjustments, Places such work on pharmacy

intervention program for DOP to review. Generates high numbers of cost saving

interventions.

Monitors Drug use evaluations daily making recommendations when medications are used

outside established criteria for use. Places info on shared drive/binder daily.

Contributes Drug monographs and/or drug reviews for Pharmacy and Therapeutics

Committee providing cost efficacy analysis.

Provides nutrition support therapy with the nutrition support team. Records patients

duration and indication for TPN in shared drive. Reports to quarterly meeting of

Nutrition support service with clinical dietician

MINIMUM QUALIFICATIONS

  • Education: Bachelor or Doctorate of Pharmacy;

Experience: Pharmacy experience preferred

  • Required Licensure: Licensed Pharmacist or pending licensure for new graduate
