PeaceHealth LPN Chronic Disease Coordinator in Florence, Oregon

PeaceHealth is seeking a LPN Chronic Disease Coordinator for a Full Time, 1.00 FTE, Day position. The general salary range for this job opening at PeaceHealth is $27.47 – $41.21. The actual hiring rate is dependent upon several factors, including but not limited to, the job/position responsibilities, location, terms of the applicable collective bargaining agreement, education, training, work experience, seniority, work performance, etc.

DETAILS OF THE POSITION:

  • Coordinates and integrates care of selected populations by managing chronic disease registries and conducting outreach using various program delivery methods. Evaluates reports and acts on outlier data based on criteria set forth by the PHMG Quality Committee, appropriate regional disease management programs and/or performance improvement committee(s).

  • Under care manager supervision, case manages a panel of patients, including implementing care plans and utilizing tracking tools, coaching for activation, providing education, patient/family support and coordinating with other care providers and resources.

  • Works with discharge planners and coordinates transitions of care, arranging warm handoffs between the clinic and hospitals, emergency departments or care facilities. Facilitates the sharing of information across people, functions, and sites.

  • Performs initial health screening per established protocols, resolves patient inquires within scope, and escalates patient inquiries beyond scope to RN Care Manager via warm handoff.

  • Participates as a proactive team member of a patient-centered medical home.

WHAT YOU BRING:

  • Accredited Program Required: Licensed Practical Nurse.

  • Minimum of 2 years Preferred: Experience in an outpatient care setting for LPNs.

  • Preferred: Familiarity with desktop computing tools, multi-line phone systems and insurance billing.

  • Preferred: Experience or training in motivational interviewing or coaching for activation.

  • Required: Licensed Practical Nurse - Oregon.

  • Required Upon Hire: Basic Life Support

  • Ability to maintain patient privacy and confidentiality. (Required)

  • Ability to collaborate with other multi-disciplinary team members. (Required)

  • Proficiency in the use of computer and office equipment, including Microsoft Office applications. (Required)

  • Strong interpersonal and communication skills including basic understanding of motivational interviewing techniques. (Required)

  • Strong writing skills. (Required)

  • Effective telephone communication skills. (Required)

  • Strong customer service skills. (Required)

  • Ability to adhere to quality, safety and infection control standards. (Required)

  • Ability to manage personal growth, taking initiative to meet challenges within the team care environment. (Required)

  • Effective prioritization skills. (Required)

  • Familiarity with desktop computing tools, multi-line phone systems and insurance billing. (Preferred)

  • Ability to work in an independent and remote environment as position may be a hybrid of onsite and remote work.

  • Ability to work from home in a secure environment free of distractions with appropriate high-speed connectivity

  • Valid drivers’ license. Responsibilities may include home visits as needed and/or travel to and from clinic sites at multiple locations.
