FOUR SEASONS HOSPICE RN Case Manager (Registered Nurse Case Manager) in Flat Rock, North Carolina

RN - Case Manager - Macon and surrounding counties

Regular Full-Time RN Franklin, NC, US Salary Range:$28.64 To 40.32 Hourly

Reports To: Clinical Team Leader

Status: Exempt - Salaried, Full Time

Shift: 40 hours; Monday - Friday, 8AM-4:30PM

Location:This position will travel to see patients in our West Region - Cherokee, Macon, Swain, Graham, Jackson, and Clay.Mileage Reimbursement Included.

Position Summary:

The Primary Nurse coordinates the nursing plan of care, provides direct nursing care, and instructs/supervises the LPN and Hospice Aide staff in the delivery of nursing care to clients.

Manages the nursing care for an assigned caseload of approximately 15 patients with varying acuity and needs. Caseloads above this target will be co-managed or additional support will be offered. In the event the assigned caseload falls below this target, the nurse may be asked to provide support to other team members.

Essential Duties and Responsibilities:

  • Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation (SeeComponents of Nursing Practice for the Registered Nurse{target="_blank"})
  • Assess the patient's physical and mental health including the patient's reaction to illnesses and treatment regimens
  • Record and report the results of the nursing assessment
  • Plan, initiate, deliver, and evaluate appropriate nursing acts
  • Teach, assign, delegate to or supervise other personnel in implementing the treatment regimen
  • Collaborate with other healthcare providers to develop the appropriate patient and family-centered nursing plan of care for the patient and the family
  • Ensure coordinated implementation of the nursing plan of care
  • Implement the treatment and pharmaceutical regimen prescribed by any person authorized by State law to prescribe the regimen
  • Provide teaching and counseling about the patient's health, disease progression, and symptom management
  • Report and record the plan for care, nursing care given, and the patient's response to that care
  • Supervise, teach, and evaluate those who perform or are preparing to perform nursing functions
  • Provide for the maintenance of safe and effective nursing care, whether rendered directly or indirectly (by unlicensed staff, patients and families)

  • Must be willing and able to care for patients with infectious disease processes unless approved accommodations
