The Hospice Liaison is responsible for assessing, planning and developing referrals. This position will establish and maintain professional relationships with key physician groups and referral sources, as well as act as a liaison to referral sources on behalf of Catholic Home Care and Good Shepherd Hospice and identify potential opportunities for market growth and program expansion. The ideal candidate will possess excellent interpersonal, marketing and customer service skills, and act in a professional and supportive manner at all times. Minimum Requirements: Current and valid NY State license to practice as a Registered Nurse Minimum of one year acute care hospital experience required Proficient with Microsoft products Two years hospice experience preferred Bachelor's degree preferred Salary Range USD $20.00 - USD $40.00 /Hr. This range posted serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies, and experience and position location.
