Job Summary:

Trinitas Emergency Department is one of the busiest in the state with more than 73,000 emergency visits annually. Our newly renovated Emergency Room has a total of 45 treatment rooms with separate treatment areas for families, children and seniors.

The Licensed Practical Nurse (LPN) functions under the supervision of a professional nurse and the nurse manager in providing patient care utilizing the scope of practice defined by the Nurse Practice Act and regulatory standards. Perform specific duties and are considered an integral part of the nursing team.

Performance Improvement:

Actively participates in meetings, departmental projects and activities as required to improve quality of

service.

Demonstrates knowledge of how to identify and report a sentinel event.

Creates and promotes an environment conducive to teamwork and improving performance.

Maintains flexibility to adapt to a variety of workload assignments.

Participates in problem solving and decisionmaking process to enhance patient care and unit function.

Contributes to a culture of excellence by demanding self motivation, adaptability, and reliability.

Care of the Patient:

Administers medications accurately and safely, monitoring therapeutic responses, adverse reactions and

incompatibilities.

Promotes and implements the concept of patient and family centered care.

Practices within the policies and procedures of Trinitas Hospital and the Nursing Service Department.

Works with the RN to establish and implement a plan of care.

Assists RN in pain assessment and selection of strategies for pain management control.

Responds to patients needs / findings in a timely manner.

Delivers safe, goal directed care that prevents complications, i.e. falls, decubiti, aspiration, and hazards of

immobility.

Provides comfort measures to patient by implementing pain management strategies.

Adheres to interdepartmental specific restraint policies and implements a safe and least restrictive

environment.

Recognizes and responds to patient emergencies, documents significant change in patient condition and

patient response to interventions/treatment and notification of RN.

Draws lab specimens and assures that appropriate labeling associated with each specimen is correct,

complete and sent to lab in a timely manner.

Complete tasks appropriately as delegated by the professional nurse.

Assists other health care workers with treatment and procedures as directed by the RN.

Understands and incorporates cultural diversity into all aspects of patient care.

Provides direct nursing care as outlined in the LPN Standards of Practice.

Collaborates with ancillary personnel to ensure quality of care services.

Performs other duties as required.

Reports patient s response to treatment to RN or MD.

Implements appropriate prevention skin care measures to maintain skin integrity.

Education:

Collaborates with the RN to create and reinforce the educational plan of care.

Documents patient response to educational activities.

Assists RN in identifying patient/family readiness to learn, taking into consideration language, and cultural,

spiritual, and physical barriers to learning.

Documents patient and family response to education about the causes and risk factors for pressure ulcer

development and ways to minimize risk.

Continuum of Care:

Assists the RN in the completion of pertinent information necessary to appropriate individual and/or

departmental agencies that will provide the next level of care.

Consults with the RN in recognizing and removing barriers to promote progress along the continuum.

Assists the RN in inventories for controlled substances according to nursing policy and procedures.

Patient Safety Competencies:

Medical/healthcare error/risk reporting.

Infant abduction prevention and response.

Interdisciplinary collaborative team approach to patient care planning.

Fall prevention measures in Emergency Department.

Restraint alternatives.

Restraint patient monitoring.

Medication Safety for Emergency Department.

Emergency Department suicide risk factor.

Additional Requirements:

High School Diploma Required.

Graduate of accredited Licensed Practical Nursing program.

Valid New Jersey LPN License.

BLS Healthcare License Current.

Good oral and written communication skills.
