Dayton Children's Hospital Care Management Coordinator-Integrated Care in Dayton, Ohio

Facility:Dayton Children's - Main CampusDepartment:Integrated CareSchedule:Full timeHours:40Job Details:The Care Management Coordinator functions as a member of a multidisciplinary health care team with guidance of the nurse case managers, utilization review, and social workers, assisting in facilitating patient post-acute placement needs; utilizes hospital and community resources for continuity of patient care; functions as a hospital liaison with external third-party payors for denials prevention; collaborates with medical staff, patient registration, HIM and other hospital departments and staff as required. In addition to performing the essential functions listed below, may also be assigned other duties as required.

Back office activities require interaction with individuals and teams inside and outside the department and facilitating communication between them. Work is consistent with the mission, strategic plans, and policies of the organization, and will be conducted in collaboration with other departments to ensure appropriate and efficient usage of resources.

Department Specific Job Details:

Education

  • Associate degree from accredited institution of higher learning required.

  • Bachelor's degree preferred.

Experience

  • 1-2 years Customer Service Experience required

  • Experience in acute care hospital or clinic preferred.

  • Experience assisting case management and/or denials prevention programs preferred.

Skills

  • Exceptional oral and written communication skills required

  • Team coordination skills required

  • Computer literate, proficiency in Microsoft Word, Excel, PowerPoint required

  • Epic Systems experience preferred.
