Description:

  • Description of Duties Nursing Process/Plan of Care• Provides total, comprehensive care to patients by applying the nursing process in an organized and systemic manner according to hospital policy.• Communicates proactively to the health care team, plan of care, changes in patient condition, the patient progress, the discharge plan and other relevant information.• Completes admission according to policy utilizing a multi-system approach for the assessment and reassessment.• Documents assessment/reassessment, plan, interventions and outcomes according to policy and interprets and utilizes clinical data in the plan of care: - Documents pain assessment and plan of care. - Documents fall risk and plan of care - Documents wound risk and plan of care. - Documents suicide assessment and plan of care. - Documents infection control assessment (MDRO protocols).• Performs accurate and thorough nursing physical assessment and documents thoroughly.• Reassesses patient condition per policy.• Interprets & utilizes clinical data in the plan of care.• Assesses patient for potential risk factors.• Recognizes and integrates patient special needs and individualizes care.• Includes pain management and supportive care issues in plan of care.• Adheres to infection control standards specific to patient care.• Assumes accountability for managing the delivery of patient care.• Responsible for learning policies and nursing procedure and communicates to others.• Performs accurate and thorough nursing physical assessment.• Participates in discharge planning, using a multi-disciplinary approach. Participates in multidisciplinary rounds.• Assists patient/family with grieving process and end of life cases as appropriate. Medical Surgical/Telemetry Nursing Skills • Wound prevention and care• Pressure ulcer• Arterial and Venous stasis ulcer• Surgical wounds• Initiates IV therapy as per policy/procedure, taking into consideration vein condition, patient age, skin condition, patient positioning, and cognition• Monitors and maintains venous access:• Vesicants • Non-vesicants• Documentation• Demonstrates care and maintenance of central venous access devices• Dialysis Therapy responsibilities • Pre and Post hemodialysis• Peritoneal dialysis• POCT: Blood glucose Monitoring & Quality Control • Provides respiratory support (Pulse ox, suctioning, wall and portable oxygen therapy)• Maintains drainage tubes• Problem focused physical assessment• Interprets cardiac rhythms and responds appropriately• Cardiac monitors• Customizes/edits/reviews patient alarms/information• Trouble shoots poor waveforms and equipment problems• Assigns transmitters when necessary• Notifies supervisor/biomedical of equipment/ failure/problem• Safe Handling of the Post Chemo patient• Phlebotomy skills• Blood culture, T&C specimens, etc.• Central venous access device• Labeling of specimens. Requirements: BSN or BS/BA degree in Nursing Current RN Pennsylvania state license required. Current BLS required or obtained by completion of orientation. ACLS required Professional certifications preferred.Strong interpersonal and communication skills. Candidate must be organized and able to lead and manage a team. Demonstrates ability to use problem solving, critical thinking and priority setting skills. Ability to perform diversified duties with time limitations with a high degree of accuracy. Computer skills preferred.Prior clinical experience preferred.
