Mercy Fitzgerald, a member of Trinity Health Mid-Atlantic, is looking for an experienced PT to join our Acute Care team!

Shift:

1FTE, 40hrs/wk Monday-Friday, rotating weekends. Per department needs.

Summary:

Performs evaluations and implements treatment in all levels of care (acute, TCU, acute rehab, outpatient). Delivers care in accordance with patient practice guidelines upon physician referral and under the supervision of the Senior Therapist. Adheres to all hospital and department policies including but not limited to: infection control, performance improvement, billing procedures and scheduling. Assists in the supervision of Physical Therapy Assistents, students, aides and volunteers.

Major Responsibilities:

  • Evaluates referred patients for need of Physical Therapy services. Is aware of current guidelines for appropriateness of acute rehab admission, skilled nursing admission, transitional care setting, home care needs and outpatient needs and documents clearly in evaluation and progress notes.

  • Performs and records patient screens, evaluation and reevaluation per department policy. Does a thorough chart review.

  • Establishes realistic treatment plan with measurable and achievable goals with stated time frames.

  • Designs and implements treatment programs to achieve goals and functional outcomes.

Requirements:

  • Bachelor's degree in Physical Therapy. Masters degree or Doctorate preferred

  • Must be a licensed Physical Therapist in the state of PA

  • BCLS certification required

  • 2-3 years of acute care experience
