Full time RN GYN Oncology-Perinatology Navigator- Danbury, CT Monday- Friday 8:00am-5:00pm in Danbury, CT

Summary:

The Gynecology & Perinatology Nurse Navigator assists patients with complex gynecological or maternal/perinatal conditions as well as their families to offer the most effective, efficient, and coordinated health care by utilizing internal and community-based resources. Collaborates with other members of the interdisciplinary healthcare team to provide uninterrupted and integrated medical, financial, psychosocial, and vocational care of the patient in accordance with the patient’s wishes, through a process of assessment, planning, implementation, and evaluation. Assists by working directly with patients to offer timely coordination of care, consistent support, education regarding care plans and treatment options, and assistance coordinating and scheduling tests and appointments. Communicates with all healthcare team members, support services, and family members to ensure the patients receive value-based care. Works with culturally-diverse populations and serves all patients regardless of their income, race, or insurance status.

Responsibilities:

  1. Serves as a point of contact for providers, patients, and families to assist with accessing clinical and supportive care services within the Nuvance Health system and in the community

  2. Develops and maintains close relationships with all perinatology patients and all referred gynecologic patients, as well as all care team members

  3. Provides patient/family education and explanations about new diagnoses

  4. Interacts with office staff to ensure proper scheduling of office visits

  5. Provides reminder calls prior to appointments for perinatology patients with high no-show rates

  6. Communicates with providers regarding procedural outcomes, patient progress, and care plans

  7. Contacts patients to assist in setting up appropriate internal and external referrals

  8. Assesses patients’ clinical, emotional, and other related concerns to overcome issues with language, literacy, and other challenges

  9. Communicates with patients and the care team members to identify barriers to care (transportation, unemployment, childcare, etc.), to help facilitate patient care, and to set goals for care based on each patient’s identified needs

  10. Assists patients in connecting with appropriate supportive care services, including behavioral health, social work, transportation, medication assistance, WIC, SNAP, etc.

  11. Acts as a liaison to coordinate consultations, referrals, and transfers of care (prenatal genetic testing, Children’s Hospitals/Neonatal Intensive Care Unit, pediatricians and pediatric specialists, endocrinologists, diabetes education, The Heart Center for hypertension or pre-eclampsia in pregnancy, pelvic floor physical therapy, urogynecologists, primary gynecologists, etc.)

  12. Documents accurate patient records in the navigation summary in the EMR

  13. Participates as a member of the Gynecology & Perinatology Team; achieves the organization’s established expectations with regard to customer service, teamwork, safety, and self-development.

  14. Maintains and models Nuvance Health’s Foundational Values (Personal, Imaginative, Agile, Connected)

  15. Fulfills all compliance responsibilities related to the position

  16. Travels between different sites as necessary

  17. Performs other duties as required

Education Skills Experience:

Registered Nurse- State of CT

Nursing Degree

Experience in Women’s Health/Case Management

Minimum Experience: Three years

Bilingual Spanish: Preferred

Working Conditions:

Physical Effort: Medium to Heavy effort. May exert up to 35 lbs. force
