AMERGIS Healthcare Staffing is seeking a Manager of Complex Care Coordination who is responsible for managing and leading a multidisciplinary care team composed of care coordinators and community health workers to provide effective, efficient care coordination. Leads the day to day activities of the care coordinator's, including scheduling, case assignment, mentoring and problem solving.

Responsibilities:

• Maintains full caseload, complete with ongoing individual service plans

• Assess ongoing changes in behavior, circumstances, or conditions that may affect patient safety

• Engages /involves client and sponsor in the casework process

• Prepares, compiles, and submits accurate case files

• Serves as a liaison with stakeholders (i.e.. legal providers, immigration court)

• Consults with supervisors to determine decisions as they relate to the safety, well-being and permanency of patient

• Maintains accurate records, files, forms, statistics and additional relevant information in accord with agency policy, licensing, and/or funding requirement

  Minimum Requirements:

• Master's Degree in accredited college or university

• MUST HAVE 1ONEof the following: LSW, LCSW or RN

• Three years of health plan experience in Medicaid or other government lines of business or health system including aspects of operations and oversight and management.

• Three years of experience with Medicaid Managed Care role with direct product and/or direct program experience in contract compliance, government relations and/or provider network.

• Two years of experience managing or supervising staff for a Managed Care Organization

• Prior experience supporting grant management

• Experience with new program implementations and oversight

• Proficient with Microsoft Office products
