Job Description:

Enrolls new members to meet established growth targets for MLTCP. Provides ongoing case management to assigned members. Promotes awareness, understanding and visibility of the MLTCP among potential members, and those with influence over health insurance plan selection. Works under general supervision.

Minimum or Preferred Qualifications:

Associate of Applied Science in nursing required. Bachelor’s degree in nursing, or the equivalent preferred. 2 years of experience in nursing preferred. License and current registration to practice as a Registered Professional Nurse in New York State required. UAS (Unified Assessment System) Certification required. NYS Driver's License required. Case Management Certification preferred.

$82,223-$82,223 annually Wage will be determined based on factors such as candidate's experience, qualifications, internal equity, and any applicable collective bargaining agreement.
