Job Summary

Assists in planning and providing a comprehensive program of activities designed to meet the special needs of the residents in the LTC, ADC, and SNF units. Leads both group and individual activities.

Essential Duties and Responsibilities

• Assists in the development and implementation of recreation programs to provide social, creative, physical, spiritual and mental stimulation.

• Documents daily on resident/patient's progress and/or participation.

• Confers with the Activity Director in regard to resident's strengths and weaknesses, progress, needs.

• Prepares materials to notify residents and staff of schedules and coordinates the schedule in the best interest of the patient/resident.

• Assists in the compilation of the LTC Newsletter.

• Offers individual therapy programs to meet the needs of residents who are regressed mentally and/or physically and are unable to pursue satisfying and beneficial interests on their own.

• May assist as needed in monitoring health status of the client and administering medications as ordered by the personal physician.

• May assess SNF patients and devise an appropriate treatment plan.

• May attend weekly team meetings and report participation and needs of the patient.

• Maintains availability of supplies for activities and has resources available that are age appropriate.

• Attends all mandatory meetings and programs.

• Performs other duties as necessary or assigned.

• Complies with the Corporate Compliance Policy and all laws, regulations, and Standards of Conduct relating to the position and agrees to report any suspected violations of law or Standards of Conduct.

• Complies with all applicable state and federal regulations.

• Maintains a safe work environment using safe work practices, utilizing standard precautions and complying with hazardous waste procedures at all times as described in St. Anthony's Safety Manual.

• Maintains the confidentiality of patient, resident, employee and organizational information.

Supervisory Responsibilities

This job has no supervisory responsibilities.

Qualifications

Language Skills

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before customers, patients, families and employees of the organization.

Position Requirements, Licenses, Registrations, Certifications

• CNA certification or willingness to obtain certification. High School Diploma or GED.
