TheRegistered Professional Nurse (Mobile Medical Unit) is responsible for coordinating and delivering patient care utilizing the nursing process. The Registered Nurse participates as a member of the interdisciplinary team providing support and information about nursing services. The RN Coordinates care between other professional disciplines as needed. Through documentation and verbal communication, essential information is shared with health care team members. When care is not provided directly by the Registered Nurse, the RN assigns/delegates care to other qualified members of the health care team. This position includes no managerial or supervisory responsibilities.

Salary is $90,000/annually

KEY ESSENTIAL FUNCTIONS:

  • Supports and promotes the philosophy, objective, policies and procedures of Acacia's PROMESA INC. Nursing Department. Reviews medical policies related to patient care and dispensary.

  • Participating in planning, development and organization of medical services provided.

  • Participate in preparation and facilitation of regulatory agency site visits to include Drug Enforcement Agency, OASAS, Joint Commission/SAMHSA and NYS Department of Health.

  • Knowledge of Federal, State and City regulatory requirements and ensures all aspects of clinical care conforms to these requirements.

  • Works closely with medical staff supervised to ensure completion of performance goals.

  • Maintains compliance with regulatory and licensure guidelines.

  • Follow accepted standards of nursing practice in dispensing methadone and utilization of equipment.

  • Dispense appropriate methadone dosages, complete logs in accordance with clinic and DEA regulations

  • Demonstrate knowledge of appropriate documentation, particularly accurate documentation of all dispensing activities in a timely manner.

  • Document pertinent information of all methadone dispensing activities

  • Accurately records patient information in the electronic medical records system and patient medical record.

  • In accordance with agency protocol and Joint Commission standards, maintains adequate infection control standards.

  • Act as a liaison in coordinating patient care activities with nurses, ordering prescribers/physician and clinical staff.

  • Reviews and reports all medical accident/incidents related to patient care and provides input into actions necessary to resolve problems.

  • Participation in Interdisciplinary Care Planning process.

  • Participate in weekly and on an “as-needed basis” case conferencing sessions with multidisciplinary team members to discuss preventative measures to adverse changes as well as patient progress related to following up on their medical needs.

  • Meets with scheduled patients for health assessments and reviews scheduling with nurse supervisor daily.

  • Provide medical assessment, consultation and referrals that facilitate comprehensive and coordinated patient care.

  • Provide crisis consultation and intervention on medical emergencies.

  • Assess the patient's need for hospitalization and consultation with patient's physician(s) to include other care providers the patient is linked with and/or referred to.

  • Provide general nursing services within scope of license and patient safety goals.

  • Keeps the ordering prescribers and nurse supervisor, informed of facility problems that need concern and any recommendations concerning medical services.

  • Will greet patients and prepare them for physical examination and treatment

  • Will conduct health assessments on patients and make appropriate referrals and linkage on an individualized basis

  • Provides health education as indicated after assessing the learning needs of the patient, staff members and students

  • Assess the patient's physical and psycho-social condition on an ongoing basis throughout the continuum of care.

  • Utilize manual and automated information systems to improve efficiency and effect productivity.

  • Demonstrate a current working knowledge of equipment, instrumentation and medications

  • Adhere to the standards, policies, procedures and goals of the medical department.

  • Act as a liaison in coordinating patient care activities with nurses, physicians and allied health professionals.

REQUIREMENTS:

  • BS Nursing

  • 3 years related experience

  • Nursing and substance abuse program experience

  • Licensed and currently registered as a Professional Nurse in New York State

  • Current CPR Certification

  • Infection Control Certification
