    Physical Qualifications:

    1. Requires full range of body motion including handling and lifting patients, manual and finger dexterity, and eye-hand coordination.

    QUALIFICATIONS REQUIRED:

    • Must be 18 years of age.
    • Must not be designated by an HHSC case worker on HHSC's Authorization for Community Care Services form as "Do not hire."
    • Must be able to demonstrate competency to perform caregiver tasks above.

    SKILLS AND ABILITIES:

    • Demonstrated ability to read, write and communicate effectively.
    • Able to document according to agency standards.
    • Ability to complete service delivery record (timesheet).
    • Ability to follow specific directions related to personal care tasks and light housekeeping duties.
    • Availability of personal, reliable transportation.
    • Able to work in various and stressful environments.
    • Able to relate well with clients and families.
    • Able to work as scheduled and report to work precisely at the scheduled time.
    • Able to lift a minimum of 25 pounds.
    • Able to push and pull up to 50 pounds.
    • Able to stand or sit for prolonged periods of time.
    • Able to perform frequent bending, stooping, or stretching.
    • Able to analyze and handle emergency situations.
    • Able to manage time wisely and complete tasks daily.

    MAIN FUNCTIONS:To perform personal care tasks and household work related to

    the care of the aged or infirmed individual (client)

    ___________________________

    ESSENTIAL TASKS:

    1. Assist the Client with the following Personal Care Routines:

      1. Mouth care, denture care
      2. Grooming (shampoo, dry and comb hair, shave, ordinary care of nails)
      3. Bathing (bed bath, tub bath, shower)
      4. Elimination (bedpan, commode, toilet)
      5. Transferring (bed to chair, a bed to wheelchair)
      6. Meals-prepare, serve, assist with feeding as needed) a well-balanced diet
      7. Dressing and undressing activities
      8. Self-administered medications (remind the client to take medications)
      9. Ambulation and movement (assist to avoid falls, safety)
    2. Perform household tasks essential to the client's needs as they relate directly to the client's comfort and well-being:

      1. Light housekeeping
      2. Make beds, change linens
      3. Dust and vacuum the client's room
      4. Tidy kitchen, bathroom
      5. Wash dishes after meals
      6. List needed cleaning supplies for the home (paper goods, also)
      7. Wash client's laundry
      8. Shopping, opening mail, banking, and errands if no other management is possible.
    3. Perform other health-related tasks which may be delegated by a physician, an R.N., or other healthcare professionals.

    4. Advise the Agency staff of any incidents in the home and/or changes in the client's status.

    5. Other reasonable, acceptable tasks which may be requested by a client who is self-directing, or the client's family. These tasks may include observation of the client, recording and reporting facts about the client's behavior, and carrying out appropriate procedures and techniques, as appropriate.

    6. Ensures that the rights of the client are maintained.

    7. Reports to work and works as scheduled and assigned by the client and field supervisor.

    8. Reports to the Coordinator at the office in advance of ch nges to the schedule or absence from work.

    9. Attends staff conferences and in-service meetings as scheduled.

    10. Represents the Agency in a loyal manner and dresses in appropriate clothing.

    11. Completes service delivery record (timesheet) on the day of each visit and turns in bi-weekly time sheet as soon as possible after the last day worked during the pay period

    12. consistently and accurately record time worked for each clients scheduled hours.

    13. Other duties as assigned.
