United Health Services Nurse Care Manager in Binghamton, New York

Nurse Care Manager, UHS Binghamton General Hospital United Health Services is seeking a Nurse Care Manager to join our BGH Care Management department at UHS Binghamton General Hospital in Binghamton, NY. This is a Per Diem position.

Overview

Facilitates patient care and patient transitions through the health care system while managing the timely and appropriate use of resources. Collaboratively assesses, plans, coordinates, monitors and evaluates patient treatment plans to promote effective utilization of services throughout the continuum of care and to ensure quality outcomes. Monitors the patient care process and collects data to evaluate and improve care and collaborates with physicians and other health care workers to provide comprehensive care including health risk appraisal, health promotion and clinical outcome studies. Educators' payers, providers and the public and serves as the information source regarding state and national health care trends; initiatives, which enhance the provision of, care and ensure efficient resource utilization.

Qualifications

Education/Experience

Minimum Required:

  • Associate in applied science in Nursing with a minimum of five years of experience in acute/tertiary facility.

Preferred:

  • Bachelor’s of Science in Nursing with 3 years of experience in acute/tertiary facility.

License/Certification

Minimum Required:

  • NYS Registered Nurse
