The Advanced Practice Nurse (APN) or Physician Assistant (PA-C) Inpatient provides advanced nursing care in the acute care setting, based on educational preparation and/or certification in the APP role to a specific patient population. Care goals include promotion of healing, prevention of illness and recidivism. The APP assumes the management of patients, functioning in both an independent and collaborative role with the physician, case manager, and other members of the health care team. As an expert practitioner, the APP performs selected invasive procedures, designs individualized treatment plans, evaluates outcomes, and modifies the treatment plan as needed. The APP facilitates the transition of patients between service units and provides/coordinates complex patient and family teaching.

  • Ensures that care delivered to patients, meets/exceeds standards of care and is provided according to the mission, strategic plans and all internal/external regulations, policies, and procedures. Practices within the Illinois Department of Professional Regulation Nurse Practice Act. Supports the established care models.

  • Functions independently and collaboratively with the physician to provide continuity of health care services for individuals, families, or groups.

  • Evaluates the health status, acute episodes of illness, and health care needs of individuals through emphasis on comprehensive history, physical exam, and psychosocial assessment.

  • Analyzes data to identify health/illness problems, the human responses to actual or potential health/illness problems, identifies variations from the norm in diagnoses emergent problems and complications.

  • Designs and evaluates individualized treatment plan. Consults with other members of the health care team on emergent problems for rapid disposition/resolution.

  • Coordinates care for optimal continuity of care across the health care continuum.

  • Facilitates patient/family participation in health care by identifying learning needs and providing specialized education to enhance decision making.

  • Exhibits professionalism by performing research, education, and leadership in clinical practice.

CPR – Basic Life Support

ACLS

Nursing – Current Registered Nurse License - State of Illinois

Other: Specialty certification in a clinical or functional area of nursing granted by a national nursing organization is preferred
