As a professional practitioner, the Telemetry LPN/LVN assumes the responsibility and accountability for delivery of patient care of telemetry patients. The Telemetry LPN/LVN demonstrates the ability to make clinical judgments in an effective and efficient manner, with supervision. The Telemetry LPN/LVN demonstrates critical thinking and performance ability in the coordination of patient care, and consistently performs according to nursing standards while remaining accountable for providing patient care and assisting others in the management of patient care.

Essential Duties and Responsibilities:

• Utilizes the nursing process to assess, plan, implement, and evaluate patient care on telemetry unit

• Performs focused interviews to identify specific patient needs

• Assesses signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition

• Monitors EKG output and keeps doctors informed of any changes

• Records vital signs, including oxygen levels, blood pressure, breathing patterns, etc.

• Monitors patient progress and updating patient medical records as needed

• Responds to patients experiencing cardiac pain, notifies RN and physician if applicable

• Manages and administers medication

• Monitors and uses the EKG

• Educates patients on cardiac health and communicates with patient’s family when applicable

• Administers tests, analyzes lab results, then reports to the physician

• Documents the patient’s plan of care

• Identifies and prioritizes actions based upon patient care requirements

• Performs interventions according to identified priorities, plan of care, and the hospital policies and patient care outcome standard

• Provides a safe, comfortable and therapeutic patient environment

• Documents patient’s response to nursing and other interventions

• Monitors safety and effectiveness of the environment and equipment

Minimum Requirements:

• Current Licensed Practical/Vocational Nurse license for the state in which the nurse practices

• One year experience in Telemetry preferred

• Complies with all relevant professional standards of practice

• Participation and completion of Amergis' Competency program when applicable

• Current CPR if applicable

• TB questionnaire, PPD or chest x-ray if applicable

• Current Health certificate (per contract or state regulation)

• Must meet all federal, state and local requirements

• Successful completion of new hire training as applicable to job site

• Understand patient confidentiality and HIPAA requirements

• Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required

• Computer proficiency required

• Must be at least 18 years of age
