Position Summary:

Provides safe, effective, appropriate and cost-efficient medication therapy to patients in the area of Infectious Disease. Assist in planning, organizing, and monitoring of the delivery of pharmacy services for RGH as it pertains to infectious disease under direction of the Director of Pharmacy Services. Reports to: Pharmacy Director

Essential Functions:

Performs duties of Pharmacy lead in hospital's Antibiotic Stewardship program

Collaborate with infectious disease physicians, pharmacy clinical directors/managers, infection prevention, clinical laboratory staff and other professionals to develop an antimicrobial stewardship program (ASP).

Develop interventional strategies to maximize appropriate antibiotic utilization and improvement in patient outcomes including but not limited to antibiotic guidelines, clinical pathways, restriction criteria, policy development, new laboratory methods and formulary management.

Provide ongoing education to pharmacists, physicians, nursing and other disciplines.

Applies knowledge of age-related factors appropriately to patients, utilizing appropriate policies and procedures.

Performs other duties related to the practice of pharmacy.

FLSA: Non-Exempt

Minimum Education Graduate of an accredited College of Pharmacy Program Required

Doctor of Pharmacy Preferred

Required Skills Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

Required Licenses [/West Virginia, United States/] /Pharmacist/ Current licensure in state of WV to practice pharmacy. Board certification (BCPS or CCP) preferred but not required.

Minimum Work Experience Hospital Pharmacy Practice and Clinical Pharmacy practice experience in an Infectious Disease setting is required. PGY1 Residence and/or PGY2 Specialty Residency training in Infections Diseases or equivalent experience preferred, but not required. Experience with experiential training and research preferred but not required. Must be able to speak, read, and follow instructions in English.
