Bilingual candidate needed for this position to service clients.

POSITION SUMMARY: This is a specialized service position providing screening, community resource linkage, and care management for the target populations. Works closely and under the direction of a licensed provider in primary care and behavioral health setting. Performs basic assessment procedures including screeners and intake. Performs interpretation for clients and staff as applicable. Services may also include orientation, case management, patient education, referral, reporting, and record-keeping. Performs light administrative duties, such as copying and filing. Work with Project LAUCH team integrating primary care and behavioral health services. Participates in health fairs or community events on weekends as requested 3-4 times a year.

*Provides patient care and/or support activities appropriate to ages served; primarily infant and children birth-8 years, and adult caregivers of young children, ages 18-65.

EDUCATION REQUIREMENTS: Highschool diploma or GED required. A minimum of two years’ college work in related human services field preferred.

EXPERIENCE REQUIREMENTS: A minimum of 2 years related experience working with mental health consumers. Bilingual preferred. Experience and knowledge of OSHA, HIPAA and CLIA guidelines preferred.

JOB KNOWLEDGE: Must possess proven professional and cultural competence, ability to work independently and to function accountably within a team. Know how to carry out mental health recovery and support services (as noted above) and participate in planning and implementation of organizational goals and objectives. Must understand importance of collaboration and demonstrate ability to develop and maintain positive partnerships that enhance service delivery. Computer knowledge is desirable. Bilingual written and verbal skills may be preferred in some positions. The ability and legal right to drive in Georgia, including an active, current Georgia Drivers License, may be required.
