Social Worker, LMSW / ED Patient Navigator / Care Transition Manager

Work location: Texas Health Arlington Memorial, 800 W. Randol Mill Road, Arlington TX 76012

Work hours: Full Time, Days, 36 hours weekly, (3) 12 hours shift, Sunday through Tuesday.

Here’s What You Need

  • Master's Degree in Social Work is required. (Individuals hired as a CTSW prior to May 11, 2017, will be grandfathered to the CTSW position with BSW, at the entity they were employed at on May 11, 2017.)

  • LMSW - Licensed Master Social Worker or LCSW - Licensed Clinical Social Worker is required upon hire.

  • CPR – Cardiopulmonary Resuscitation is required upon hire.

  • Three (3) years of acute care hospital/medical social work is highly preferred.

  • One (1) year of discharge planning and/or care management is highly preferred.

  • Emergency Room experience preferred.

  • ACM - Accredited Case Manager orCCM - Certified Case Manager upon hire is preferred.

  • ANCC upon hire is preferred.

Skills for success in this role include:

A working knowledge of medical necessity criteria, the ability to engage in complex clinical decision-making and psychosocial and crisis intervention skills are required. Strong customer service skills and conflict resolution are needed. Strong oral and written communication skills; a commitment to interdisciplinary collaboration, and knowledge of Microsoft Outlook and Office (Word, Excel), are needed.

Here's What You Will Do

Responsible for ensuring patients are transitioned to appropriate levels of care and completes Transition Evaluations including interviews and assesses patients and caregivers as part of the transition evaluation and as needed. Serves as a point of contact for all identified stakeholders. Schedules PCP, specialist, or clinic follow up appointments for patients. You’ll also participate in Joint Commission and other survey readiness activities and clinical review staff requirements and communications.
