Community Health Worker/Lifestyle Coach

Per diem

The Diabetes Prevention Program (DPP) lifestyle coach will lead participants at various ministries through the year long lifestyle change program. Helps participants develop and maintain the skills needed to adopt healthy eating and physical activity habits. The Lifestyle Coach (LSC) provides support by providing information, encouraging progress, and working with the group to address any challenges or barriers that arise.

ESSENTIAL FUNCTIONS

  • The LSC will receive referrals and outreach to participants. They will send and collect intake information and will enroll or waitlist participants into their preferred time, date, virtual or in-person cohort.

  • Facilitates sessions over the course of the 1-year program (weekly sessions for 16 weeks, then bi-weekly for followed by monthly sessions for the remainder of the year).

  • Delivers the National Diabetes Prevention Program lifestyle change program as it was designed. Provides culturally relevant examples of food, types of physical activity, barriers, etc. while adhering to the key messages of the curriculum.

  • Reinforces and integrates session material and builds on concepts by referring to previous sessions and discussions.

  • LSC will collect attendance, weights, and activity minutes weekly and enter data in appropriate documentation.

  • LSC will document on each participate for each session. Records weekly data for each participant for submission to the Diabetes Prevention Recognition Program (DPRP).

  • Supports participants by providing information, encouraging progress, and working with the group to address challenges or barriers that arise.

  • Encourages goal setting on a weekly basis.

  • Prepares for each session (i.e., reviews lesson plan and content for each session, provides feedback for participants).

  • Accessible to participants before and after sessions to answer questions and follow-up on any questions that cannot be answered during class time.

  • Follow-ups with participants outside of class if they are unable to attend a class.

  • LSC will send surveys where applicable.

  • Complete Healthstream assignments as assigned.

  • Complete and send time and efforts and submit monthly to supervisor.

MINIMUM QUALIFICATIONS

  • High School Diploma or equivalent combination of education and experience to include a focus in public health, social work, business, advocacy or health administration.

  • Previous experience with electronic medical record systems is preferred.

  • Possesses strong analytical skills, leadership and team building abilities, excellent interpersonal and communication skills.

  • Ability to communicate in an open, non-judgmental, empathetic, flexible and compassionate style. Strong and effective facilitation skills. Ability to listen to and engage others in one-on-one or large group discussions.

  • Possesses a personal presence that is characterized by a sense of honesty, integrity, and caring with a willingness to support the mission, vision, goals, and values of Trinity Health.

Pay Range: $17.85-$23.80

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
