Community Support Services Inc RN - Nurse Care Coordinator in Akron, Ohio

*The Opportunity: *We are seeking to hire an ACT Nurse Care Coordinator to join our Assertive Community Treatment (ACT) Team. This individual works closely with the behavioral health provider, primary care providers, behavioral health team members, and the community agencies; provides professional assessment, coordination/planning of multiple health care services, and an ongoing evaluation of care management services to ensure that necessary clinical services are received.  The ACT Nurse Care Coordinator will process consults and deliver nursing care based on the client's competency and cognitive need, and facilitate/re-enforce education while fostering client accountability and self-management skills.

 

What You'll Do:

  • Perform required nursing assessments to identify those individuals with identifiable risk factors (at-risk in levels or blood pressure and weight/BMI risk level).
  • Act as liaison between public and private healthcare facilities, and service agencies to carry out referral instructions and coordinate referrals to medical providers, i.e. Assisted Living Facilities; Home Health Care; PASS-R, etc.
  • Participate in interdisciplinary treatment team meetings, cooperate with and utilize the resources of other community agencies as described above, including other CSS programs, in order to achieve treatment goals of individuals served.
  • Serve as liaison consultant to behavioral health treatment team on medication and healthcare issues to ensure coordination of care.
  • Incorporate measurable, disease-specific goals into treatment plans.   Evaluate progress and communicate to treatment team in order to determine which member of treatment team will deliver required services.
  • Plan and co-lead groups in support of health behaviors (smoking cessation, weight management, exercise, and nutrition).
  • Make home visits to administer and/or assess individual's condition of health or provide other interactive services away from the clinic and in the individual's environment as appropriate.
  • Provide crisis intervention, supportive interaction, and education as needed to foster self-management skills and accountability.
  • Accompany clients to medically complex medical appointments (specialists or initial primary care appointments.)
  • Receive clinical supervision from Nursing Supervisor and participate in periodic professional review which is monitored by the Nursing Supervisor. 
  • Expected to be in the community up to 75% of time.
  • This position is full-time (40 hours a week) working normal business hours.

     

What We're Looking For:

Graduate of accredited nursing program and valid license to practice as an RN in the State of Ohio required.

Experience providing nursing services to persons with severe and persistent mental illness in a community and clinic setting.

A car and valid Ohio Driver's License and insurability through the agency's insurance carrier.

Knowledge of nursing rules and regulations in the State of Ohio, including discernment regarding scope of practice issues and thorough knowledge of nursing standards and practice.

Excellent organizational, attention to detail and communication skills.

Ability to work in a team environment.

Commitment to confidentiality, following rules of HIPAA.
