Trinity Health Social Worker Part-Time Mercy Life in West Springfield, Massachusetts

Collaborates with the Social Worker II to meet the needs of Mercy LIFE participants. Establishes and maintains relationships with other departments and agencies in the community. Provides social service support to participants when they need assistance obtaining community services. Provides support and insures needs are met for participants when in hospital and short term rehab facilities. Additional social worker interventions may include participant and family education, provision of resources, advocacy to ensure that participant and family's needs are met, and disenrollment procedures.
PRIMARY JOB RESPONSIBILITIES AND DUTIES:
Demonstrates, promotes and integrates the Sisters of Providence Care Centers Mission and core values by:
In collaboration with other Social workers, acts as liaison between participants, family, and community agencies, for participant needs.
Assures that any personal care needs are communicated to the family with special attention to clothing and DME needs while participants are in hospital or short term rehab stays. Communicates relevant information to the interdisciplinary team about participants while they are admitted.
Maintains contact with participants while they are in the hospital. Coordinates hospital and short term rehab discharges in conjunction with interdisciplinary team, and by communication with attending physician.
Assists participants and caregivers with setting up and maintaining Personal Needs Accounts. Assists participants with Social Security Income (SSI) and Social Security Disability Insurance (SSDI) application process as needed.
Facilitates discharge planning based on the goals of the participant.
In collaboration with the Social Workers, may act as facilitator for meetings with participant, family and agencies, to clarify or problem solve issues regarding the Plan of Care. Mediates discussions between all parties. Assists participants and caregivers in filing grievances and appeals.
Maintains high standards of business and ethical conduct in accordance with applicable federal, state and local laws and regulations including fraud, abuse and waste. Avoids activities, which could result in perceived or actual conflicts of interest. Maintains required licenses and certifications.
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
Knowledge of Social Work principles acquired through obtaining a Bachelor’s of Social Work in order to advocate and preserve the rights of the participants.
Two (2) years' experience, including one (1) year working with the frail or elderly required. Knowledge of medical and/or geriatric social work.
Valid MA driver's license and ability to provide a reliable means of transportation required.
Ability to function with minimal supervision.
Requires work in a normal office environment where there are relatively few physical discomforts, due to dust, dirt, noise and the like.
Requires exposure to the elements while traveling between patients’ homes. Requires working in patients’ homes with varying degrees of dust, dirt and clutter.
Requires working with equipment or performing procedures where carelessness would probably result in minor cuts, bruises or muscle pulls.
May require lifting, standing, bending, transferring, stooping, stretching, walking, pushing, pulling; partial or complete assistance with activities of daily living without assistance from another health care worker or significant other.
Reports to the Center Director.
Has no responsibility for the direct supervision of any other employees.
