Mercy LIFE, a member of Trinity Health PACE, is seeking a Health Information Specialist to work part-time status for its location in W. Springfield, MA. The Program of All-Inclusive Care for the Elderly (PACE) model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible. PACE serves individuals who are age 55 or older, certified by their state to need nursing home care, able to live safely in the community at the time of enrollment and live in a PACE service area. While all PACE participants must be certified to need nursing home care to enroll in PACE, only about 7 percent of PACE participants nationally reside in a nursing home. If a PACE enrollee needs nursing home care, the PACE program pays for it and continues to coordinate the enrollee's care. We encourage fellowship and independence while serving seniors and their families with dignity and respect. Delivering all needed medical and supportive services, PACE is able to provide the entire continuum of care and services to seniors with chronic needs while maintaining their independence in their home for as long as possible.
Under the direction and supervision of the Director, Quality Improvement, implements, supervises and maintains medical records in accordance with regulatory guidelines*.* Works with others to coordinate communication among all clinical staff. General responsibilities will include:
Prepares charts for new participants. Purges records and files documents into closed charts. Files documents into active records. Sends for medical information for participant’s at time of intake and as requested by the Interdisciplinary Team.
Coordinates with the Enrollment Specialist the participant schedule for Intake and Assessment (I&A) meetings and communicates to interdisciplinary team. Develops and updates schedule for periodic reassessments of PACE participants and communicates to the interdisciplinary team.
Request, and obtains medical information for participants at time of intake and as requested by the Interdisciplinary Team, and scans documents into electronic medical records as required.
Attends I&A compiles assessment, and problem lists for new participants and periodic assessments files appropriately.
Implements and administers the electronic medical record and maintains paper medical records as required for participants in the PACE program. Works collaboratively with the electronic medical records service provider and the information services department.
Participates in orientation to the electronic medical record and provides updates and troubleshoots problems with the system as required.
Under the direction of the Quality Director and/or the Compliance Officer, releases information according to HIPAA and Protected Health Information policies and procedures. Prepares EMR records for surveys by outside agencies and other program needs as required.
Maintains paper documents as required. Files documents into active records and purges records and files.
Maintains organized electronic medical records in organized archives that ensure efficient records retrieval from electronic/shared drive folders.
Maintains a medical record form inventory (as appropriate). Reorders and restocks forms as needed.
Makes recommendations for improvements to workflows and processes as appropriate.
Candidates will have:
Education: High school diploma or GED required; Associates Degree preferred.
Minimum of one (1) year working with the frail or elderly required.
Requires a working knowledge of regulations related to medical record management.
Basic computer skills are required; ability to type a minimum of 40 words per minutes. Previous long-term medical record management experience is desired.
Requires working knowledge of Microsoft Office suite of software applications, and readiness to acquire working knowledge of other applications for internal and external reporting, monitoring, quality improvement, and department requirements.
Requires verbal and electronic mail communication, and managing electronic appointment calendar (Microsoft Outlook, etc.).
