Trinity Health Mercy LIFE Mass_Driver Part time 20 hrs days in West Springfield, Massachusetts

Mercy LIFE, a member of Trinity Health PACE, is seeking a Transportation Driver, to work part-time status, for its location in W. Springfield, MA. The Program of All-Inclusive Care for the Elderly (PACE) model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible. PACE serves individuals who are age 55 or older, certified by their state to need nursing home care, able to live safely in the community at the time of enrollment and live in a PACE service area. While all PACE participants must be certified to need nursing home care to enroll in PACE, only about 7 percent of PACE participants nationally reside in a nursing home. If a PACE enrollee needs nursing home care, the PACE program pays for it and continues to coordinate the enrollee's care. We encourage fellowship and independence while serving seniors and their families with dignity and respect. Delivering all needed medical and supportive services, PACE is able to provide the entire continuum of care and services to seniors with chronic needs while maintaining their independence in their home for as long as possible.

The Driver provides safe and timely transportation of PACE participants, including transport of participants, meals, durable medical equipment, pharmacy, and any other transportation as deemed necessary. Responsible for overall cleanliness and safety of the vehicle assigned on a daily basis. This position reports to the Transportation Manager.

General responsibilities will include:

Responsible for PACE passengers during transport, providing care in a manner which ensures participant satisfaction. Assists with outer wear, shoes and or bags of passengers.

Follows assigned schedule, makes adjustments to meet unanticipated changes, while considering the needs of the participant and requirements of the PACE Center. Communicates these changes to Transportation Coordinator or designee.

Operates equipment safely and in compliance with vehicle licensing regulations. Maintains and completes daily required records, i.e. Pre and Post Trip Inspections, gasoline purchases.

Immediately reports to Transportation Manager any vehicle or cell phone operating problems, malfunction, accidents or incidents out of the ordinary routine.

Transports participants to and from medical/specialist appointments and activity outings as requested by the interdisciplinary team via the plan of care. Provides escort to participants at ancillary appointments as needed.

Transports meals, durable medical equipment, pharmacy, and any other items as deemed necessary for participant.

Provides physical assistance to each participant when needed as s/he boards and disembarks from the program’s vehicles.

Maintains the vehicle in a neat and clean condition. Ensures vehicle equipment is maintained and operating within manufacturer’s recommendations, replacing supplies as needed.

Performs additional duties assigned in response to patient/Mercy LIFE needs.

Candidates will have:

  • Education: High school diploma or GED required.

  • Certification/License: Valid driver’s license in good standing required. Must have no infractions over the past 5 years and practice defensive driving.

  • Minimum of one (1) year working with the frail or elderly required.

  • Knowledge: Knowledge of Hamden and Hampshire Counties with the ability to interpret a map required. Ability to function with minimal supervision and ability to work in a multicultural, multi-ethnic environment. Demonstrates sensitivity to the unique needs of patients and families experiencing a serious or terminal illness.

  • Bilingual (Spanish speaking) preferred.

  • The ideal candidate will have the ability to remain courteous under stressful situations, have knowledge of utilizing a wheel chair and be capable of securing wheelchairs in confined spaces on vehicle. Excellent customer service skills are required.
