Trinity Health Mercy Life MASS_Activities Assistant in West Springfield, Massachusetts
Assists in planning, development, and coordination of appropriate social, physical, mental and educational activities to enhance and promote the therapeutic goals of holistic programs for Mercy LIFE participants. Responsible for care plan documentation and progress notes. Responsible for running and coordinating group and individual activities.
*PRIMARY JOB RESPONSIBILITIES AND DUTIES:*
Demonstrates, promotes and integrates the Sisters of Providence Care Centers mission and core values by successful completion of the following responsibilities:
Assists in planning the monthly activity calendar.
Implements planned daily activities to improve or maintain the participants’ cognitive, physical, social, psychosocial and artistic needs.
Transports participants to and from activities and other areas within Mercy LIFE.
Works as part of the interdisciplinary team to promote holistic health care for each participant.
Responsible for care plan documentation and progress notes. Conducts initial Recreational Therapy Assessments as requested and updates as required.
Plans and implements activities for dementia-specific population. Assists in maintaining daily flow of dementia activities area. Activities may focus on small groups, one-on-one programming and sensory stimulation.
Assists with organizing community outings and special events at the PACE Center. Evaluates plans to promote effective programs according to client abilities and preferences, and within the PACE Center’s daily flow of events.
Reports significant attitudes and behaviors of participants during activities to Day Center Supervisor.
Assists in notifying other departments who might be involved with the particular event/activity, i.e. working with dietician to plan event, work with rehab to coordinate groups.
*REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:*
Requires a High School diploma or its equivalent. Certification as an Activity Assistant preferred.
Minimum of two (2) years’ prior experience working with the older population and knowledge of Therapeutic Recreation and other activities preferred.
Must be able to read and write English.
Requires light physical effort on a regular basis, as in frequent standing, walking, bending, stooping or reaching and/or occasional lifting of light items (up to 20 lbs.) Pushing/pulling wheelchairs and geri-chairs.
Exposure to injury, occupational or contagious disease hazards which requires routine precautions.
May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and any other conditions common in a healthcare environment. Subject to unpleasant odors.
Rapid judgment demands and flexibility in scheduled hours. Some stress due to work with colleagues, participants and families.
Some local travel may be required.
The Activity Assistant reports directly to the Day Center Supervisor.
Has no responsibility for the direct supervision of any other employees.
