Trinity Health Day Center Worker in West Springfield, Massachusetts

Provides activities, personal care, laundry and other designated health-related services in compliance with Mercy LIFE policies and procedures, applicable health care standards, and governmental laws and regulations.

*PRIMARY JOB RESPONSIBILITIES AND DUTIES:*

Assists participants and/or supervises with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and food nail care, and toileting) as assigned or needed under the direct supervision of the Day Center Manager.

Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar and as requested by the Recreational Coordinator.

Monitors and assists participants with meals at feeding table.

Assists, throughout the day, in maintaining clean and orderly day rooms, kitchen and personal care areas.

Accompanies enrollee to medical and other appointments.

Performs and or assists with laundry, including clothes for participants.

Assists the drivers with the loading and unloading of participants from vehicles.

Reports changes in participant’s condition and needs to the Day Center Manager or Clinic Nurse.

Assists the transportation department with participants’ take-home supplies.

Acts only within the scope of his or her authority to practice.

Maintains and submits documentation as required by Mercy LIFE. Maintains confidentiality regarding participants, health care staff and documentation.

Participates in case conferences, interdisciplinary team meetings and in-service programs as required.

Performs additional duties assigned in response to patient/Mercy LIFE needs.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

High school diploma or GED required.

Must have reliable transportation.

Minimum of one (1) year working with the frail or elderly required.

Certified Nursing Assistant preferred.

Bilingual (Spanish speaking) preferred.

Ability to function with minimal supervision and ability to work in a multicultural, multi-ethnic environment. Demonstrates sensitivity to the unique needs of patients and families experiencing a serious or terminal illness.

*WORKING CONDITIONS:*

Requires work in a normal office environment where there are relatively few physical discomforts due to dust, dirt, noise and the like.

Requires working with equipment or performing procedures where carelessness would probably result in minor cuts, bruises or muscle pulls. Requires exposure to blood and various body fluids that requires caution and use of established safety guidelines and equipment.

May require lifting, standing, bending, transferring, stooping, stretching, walking, pushing, pulling; partial or complete assistance with activities of daily living without assistance from another health care worker or significant other.

*REPORTING RELATIONSHIPS:*

Reports to the Day Center Manager.

Has no responsibility for supervising the work of others.
