Community Choice Bilingual Health Homes Care Coordinator in Wenatchee, Washington

Job Summary : This full-time position located in Wenatchee, WA is responsible for providing all aspects of Care Coordination Operations (CCO) in a large geographic region served by Community Choice through the Health Homes CCO program. The job requires strong program/project management and coordination experience, as well as expertise and experience in care coordination practices that combine medical, behavioral, and social supports to improve health outcomes. The ideal candidate will be trustworthy and organized. The position requires effective outreach and recruitment of beneficiaries in a large and mostly rural geographic area, ability to work effectively with minimal supervision, and the use of Health IT platforms for documentation. The CCO Care Coordinator will become well informed with the most current information on chronic disease care management models with an emphasis on rural communities. Main Job Tasks and Responsibilities: Engage beneficiaries using information about geographic location, existing relationships with providers, and analysis of health history Coordinate program activities to ensure maximum efficiency, allocate resources to enable task performance Utilize Health IT platforms for reporting of beneficiaries' health information Ensure security and confidentiality of data per HIPPA standards Help analyze and monitor internal processes/propose procedural and policy changes to improve operational efficiency Support a safe and secure working environment Handle consumer inquiries and complaints according to set policies and procedures Represent Community Choice professionally and create relationships with partners and collaborators Desired Education and Experience: Bachelor's degree in nursing, health-related, business-related, or social service-related field Experience managing care coordination and/or collaborative initiatives, preferably related to healthcare and healthcare access Demonstrated knowledge of clerical practices and procedures High level of computer skills and an in-depth knowledge of office software packages Strong interpersonal skills, customer service skills and experience Key Competencies: Strong written and verbal communication skills Good judgment, decision making and problem solving Strong time management, planning and organizing Attention to detail and high level of accuracy Delegation of authority and responsibility where appropriate Information gathering and monitoring Effective coaching skills Initiative, multi-tasking ability, adaptability and stress tolerance Integrity, teamwork and collaboration Dependability and timeliness around regular work hours and flexibility to meet the needs of programs, partners, and clients
