Trinity Health LCSW Social Worker 40 hrs 1st shift days in Waterbury, Connecticut

Knowledge of Social Work principles normally acquired with a Bachelor’s of Social Work in order to advocate and preserve the rights of the residents. License in Social Work (LSW) is also required in order to document independently. Acts as the primary resident advocate in the facility. Documents as mandated by the DPH regulations and facility policies. Establishes and maintains relationship with other departments and agencies in the community. Provides medically related social services to meet resident needs. Demonstrates, promotes and integrates the Mission and core values by: Maintains knowledge of new admissions and assists in process, i.e., assists residents and family in completing admission packets, completes social history for those residents assigned to caseload. Acts as liaison between residents, family, and community agencies for any concerns. Participates as part of the interdisciplinary team in providing interventions to resolve behavior, mood or psychosocial problems and develops and implements plan of care.
