Trinity Health Coordinator (Part Time) in Syracuse, New York

Coordinates services to support Community Health & Well-Being initiatives. Supports program initiatives by collecting data, ensuring accuracy of the data and communicating data to system leaders. Coordinates collaborative work with internal and external stakeholders. Generates regional reports, summaries, presentations, dashboards, and graphs necessary to accurately display both participant and community level impact of the interventions deployed

ESSENTIAL FUNCTIONS

Knows, understands, incorporates and demonstrates the Trinity Health & St. Joseph’s Mission, Vision and Values in behaviors, practices and decisions.

Maintains a working knowledge of applicable Federal, State and local laws and regulations, the Trinity Health Integrity and Compliance Program and Code of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behavior.

Serves as Ministry subject matter expert on Community Benefit What Counts.

Collects and enters Community Benefit expenditures and outcomes into the CBISA software. Has confidence to assess the data provided and seek additional information when/if the data reported is inconsistent and/or doesn't meet community benefit guidelines.

Supports the development of triennial Community Health Needs Assessment.

Assists in developing and facilitating stakeholder collaborations aligned with Community Health & Well-Being activities.

Assists in implementing community benefit and CHWB programs.

Facilitates and supports program work related to needs assessments, strategic plan development, and tax reporting.

Serves as a support to the Directors of Community Health and Well-Being and Engagement and resource for stakeholders, colleagues and providers for CHWB.

Manage timely and accurate coordination for external regulatory requirements (i.e. IRS Form 990 Schedule H, federal and state requirements); populate and review Ministry Schedule H submission.

Works with the finance liaison to accurately calculate community benefit expenditures.

Regularly creates reports through CBISA and apprises Ministry CHWB management of community benefit spending and outcomes.

Manages the collection of data through multiple disparate sources to:

Create Community Health Profiles based on Trinity Health Hospital Service Area and Regional Health Ministry Service Areas

Create registries and other medical/community management reports as needed.

Analyzes clinical, community and financial data and identifies trends, potential problems and issues, followed by recommendations for action.

MINIMUM QUALIFICATIONS

Must be able to operate effectively in a collaborative, shared leadership environment.

Must possess a personal presence that is characterized by a sense of honesty, integrity and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals and values of Trinity Health.

Bachelor's degree required in Health Care Administration, Public Health, Community Health or related field is preferred.

One to two years of experience community health, public health, social services, etc.

Strong data entry skills.

Excellent communication (interpersonal, verbal and written) skills.

PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS

Must be able to adapt to frequently changing work priorities, and be able to prioritize and balance the requirements of working with the System Office and Regional Health Ministries (RHMs).

Must be able to travel to various RHM sites as needed.

Requires working in a normal office environment where there are relatively few physical discomforts due to dust, dirt, noise, and the like.

Requires spending time in other agencies and environments within the community where there may be some disruptions due to noise, interruptions and the like.

Must possess good manual dexterity in order to operate a computer keyboard.

Must possess good hearing in order to be effective on extensive telephone conference calls and for in person communication
