ESSENTIAL JOB FUNCTIONAL COMPETENCIES:

1.

Demonstrates the knowledge and skills necessary to provide service/care appropriate to the age of the patients served, in accordance with Mercy standards.

2.

In concurrence with nursing, develops guidelines and procedures for patient medication counseling and instruction. Performs patient educations instruction and prepares patient medication histories where appropriate.

3.

Provides pharmacy related educational activities and information on pharmaceutical care and drug therapy to patients, pharmacy staff, physicians, nurses, students, and other clinicians.

4.

Conducts comprehensive medication reviews, and targeted interventions to assess appropriateness of all medications including appropriate use, dose, dosage form, regimen, route, therapeutic duplication, drug allergies, and drug interactions to optimize medication therapy and improve clinical outcomes

5.

Engages in active patient-centered medication therapy management through transitional care including medication reconciliation services and discharge assistance: patient counseling on appropriate medical therapy, medication history, and facilitation of patient care, coordination of benefits, authorizations, and formulary management.

6.

Counsels patients or caregivers on medications regarding changes, side effects, and medication or food interactions, drug indication and dosing regimens.

7.

Is responsible for implementing processes (i.e. counseling, consulting, education of RN's, physicians, and patients) to reduce the risk of readmission to the ED or inpatient setting.

8.

Assists in the detection and reporting of adverse drug reactions, drug interaction medication errors, etc.

9.

Identifies and resolves potential and actual drug related problems.

10.

Provides assistance and consultation to physicians in planning patient drug regimen.

11.

Provides assistance and consultation to nursing staff in managing drug portion of patient care plans.

12.

Solves problems for medication history issues escalated by medication historians.

Provides support to the Pharmacy and Therapeutics Committee, which may include serving as a member and/or activities such as preparing formulary drug reviews, medication use evaluations, policies and guidelines for drug therapy, etc.

14.

Participates in departmental, multi-disciplinary, hospital, and health-system committees as leader, member, or consultant.

15.

Is knowledgeable of and actively participates in improving patient safety and reducing risk to patients. Complies with Safety and Infection Control Policies and Procedures.

16.

Maintains confidentiality of information pertaining to clients, physicians, employees, and Mercy business.

MINIMUM QUALIFICATIONS

Requires Phar.D. or Bachelor's degree, with at least 10 years hospital pharmacy experience in lieu of degree, and licensure or licensure eligibility in the State of Iowa. Two years hospital pharmacy, ambulatory care, or transitional care pharmacy experience preferred. Completion of a residency program preferred. Previous clinical experience preferred. Must have basic typing skills, be familiar with computers and their functions, and have experience in utilizing database programs. Must have excellent interpersonal and communication skills. Must have a general working knowledge of computers and department specific software (i.e. Healthstream, internet, etc.) Must be comfortable operating in a collaborative, shared leadership environment. Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of Mercy and CHE Trinity Health.
