Pioneer Center North (PCN) is a 141-bed facility in northwestern Washington that provides variably-intensity residential treatment for people who are struggling with chronic behavioral health issues. Located in Sedro-Woolley, PCN's programming is evidence-based for men and women. PCN accepts voluntary and involuntarily individuals committed to the program; often they have been unsuccessful in other treatment programs. SUMMARY: The Prescription Refill Coordinator supports the mission of Pioneer by coordinating access to quality and cost-effective integrated health services for clients at Pioneer facilities. This role works closely with internal staff, community providers, pharmacy staff, and provider(s) to assist in gathering and relaying necessary information to effectively assess and manage client integrated health needs. This role is responsible for receiving and preparing routine prescription refill requests for provider review and for the management of prescription assistance program applications. PRIMARY DUTIES AND RESPONSIBILITIES: Service Delivery Verifies MAR for accuracy and errors. Assists with inventory control and quality assurance ensuring client medication/supplies are adequate and accounted for, equipment and facilities meet safety and quality guidelines at all times. Maintains storage of medications, places orders and returns clients medication according to pharmacy services policies. Receives faxed and Electronic Medical Record prescription refill requests. May include controlled and/or non-controlled medications. Researches and documents information required for the provider to make an informed decision to approve or deny the refill request. Oversees the data entry and completion of prescription refill forms/notifications. Ensures requests are entered within 72 business hours of receipt. May obtain insurance pre-authorization for patients and reauthorization of medication and additional treatment. Ensures insurance carrier documentation requirements are met and refill support documentation is filed in patient's medical record. Efficiently manages correspondence with patients, physicians, specialists, insurance companies. Documents pertinent information on appropriate form and/or in medical record. May notify patients of overdue office visit and necessary screenings per refill protocol through written communication. May assist with identifying and assisting patients with applying for prescription assistance programs. Communicates with third party payers to assure coverage of patient care. Coordinates and ensures pending clients admitting to facility have all medications filled or ready prior to admission ensuring coordination and continuation of care exists. Administrative Duties Maintain accurate, timely, and complete documentation of service Ensure all necessary notifications are completed on your shift. Complete all eligibility assessments and follow-up assessments in database Ensure all client information is accurate and updated from intake to discharge for continuity of care Record routine case notes and touch points documenting progress and services Review reports with team to identify service gaps and necessary support services Collect data and reconcile missed doses, refusal and no show with EHR.
