Transitions - Mental Health Association has an opening for a Residential Case Manager. Under the general direction of the Program Manager, the Residential Case Manager provides brokerage, mental health and residential services to psychiatrically disabled adults in the community. Electronic health record documentation and inputting required in order to meet monthly contract agreements. Case Managers for the adult programs are expected to supervise the general health, safety, and social rehabilitation of clients. They also perform moderate facility maintenance, function as positive role models, promote cooperation and harmony in communal living, and uphold and promote the program philosophy in all duties performed.
Full time, 40 hours per week
Case Manager I: Bachelor’s Degree in a related Human Services field AND 6 months of experience with target population, OR AA/AS or CADAC/CPRP certification AND 2 years of experience with target population, OR 4 years of experience (Full-Time equivalent years) with target population. Case Manager II: Master’s Degree in a related Human Services field and 6 months of experience with target population, OR Bachelor’s Degree in a related Human Services field 6 months of experience with target population AND 2 years of experience as a Case Manager I, OR AA/AS or CADAC/CPRP certification and 2 years of experience with target population AND 2 years of experience as a Case Manager I, OR 4 years of experience (FT equiv. years) with target population AND 2 years of experience as a Case Manager I. Basic house maintenance skills preferred. Strong interpersonal skills and oral/written communication skills Conditions of employment include: must be at least 18 years of age; possess a valid CA Driver’s License, minimum 2 years driving experience, reliable transportation, proof of auto insurance, and DMV driving record that meets TMHA’s current vehicle insurance requirements. May be required to utilize personal vehicle and auto insurance. At the time of hire, other conditions of employment are to comply with a T.B. skin test or chest x-ray indicating a negative tuberculosis result, and receipt of a criminal record clearance through fingerprinting. Must be able to successfully complete First Aid and CPR training.
