This role will be a Telephonic RN Case Manager within Optum's Diabetes Navigator Program. This position is telecommute and ideally we are seeking someone in the Pacific or Mountain timezone.
This position will require active and unrestricted Nursing licensure in multiple US States. Selected candidate must be willing and able to obtain and maintain multiple state licensure. (Application fees and filing costs paid for by UHG)

Primary Responsibilities:
Making outbound calls to providers in the Diabetes Health Navigator program
Identifying gaps or barriers in treatment plans to close STARS gaps
Providing patient education to assist with self-management
Interacting with Medical Directors, Pharmacists, and Social Workers, Internal teams
Coordinating care for members
Making referrals to outside sources
Documenting and tracking findings
Managing the members holistically
Required Qualifications:
Active, unrestricted RN licensure in your state of residence
3+ years of experience in any clinical setting
Bilingual skills (fluency in Spanish/English)
Intermediate / proficient level of experience with MS Office applications
Must be willing / able to obtain CCM within 2 years of employment
Multiple state licensure (in addition to Compact License if applicable) or ability to obtain multiple state nursing licenses required (Application fees and filing costs paid for by UHG)
Preferred Qualifications:
Bachelor's Degree in Nursing
Experience working with diabetes patients
Clinical Diabetes treatment experience
Previous Case Management or other managed care experience
Certified Case Manager (CCM)
Experience working with the Medicare guidelines and regulations
Reside in the Pacific or Mountain timezone
