The Sr. Pharmacy Technician will perform various duties related medication adherence and medication reconciliation. The candidate will access, input, and retrieve prescription information from computer systems, providers, or patients. The candidate will perform medication history consults with patients as part of the medication reconciliation process. They will also consult with pharmacies and physicians regarding use of medications, coordinating refills, and advising on treatment plans to improve outcomes. Pharmacy Technicians must have current state license or national certification without restrictions based on regulatory requirements of worksite. Positions in this function are responsible for providing expertise and customer service support to members, customers, and / or providers. Direct phone - based customer interaction to answer and resolve a wide variety of inquiries.
Positions in this function provide clinical pharmacy advice and guidance on any or all of the following:
Utilize prescription and medical claims data as well as patient interview to generate updated medication list.
Identify and prioritize patient interviews based on workload and patient discharges
Clarify information as needed with patient’s physician office and/or local pharmacy as needed.
Coordinate with the Clinical Pharmacist to ensure that any discrepancies between medications are reviewed, evaluated and clarified
Facilitates documentation of medication reconciliation post discharge in required systems
High school diploma / GED (or higher)
State Board of Arizona Registered Pharmacy Technician (Active and in good standing) (CPhT)
Current National Certified Pharmacy Tech License (PTCB)
5+ years of experience working as a pharmacy technician
Ability to navigate MS Office and Windows - based environments MS Word (copy / paste / save), MS Excel (use spreadsheet templates, merge data), MS Outlook (send and receive email correspondence, send and receive calendar invites)
Experience with Medicare Advantage
Experience performing medication reviews/medication reconciliations
