Mercy LIFE (Living Independently for Elders, a PACE Program) has a need for a Per Diem - RN Care Manager for the Broad Street Center to work a 8 hour shift during the day.

The RN Case Manager provides oversight and management to the provision of service delivery in support of the participant-centered care model, assuring quality and maintaining open communication. Presents/represents the participant at interdisciplinary team (IDT) meeting. Collaborates with team members to assure participant care and services are delivered and needs are met. Interacts with all members of the team internal and external of the organization as needed. Works collaboratively with the Social Work partner to assure appropriate and timely care delivery.

BSN required.

Registered Nurse licensed in the State of Pennsylvania. CPR certification. Licensed to operate an automobile, and access to a vehicle, in the state of Pennsylvania. Must have prooof of automobile insurance.

Understanding of Case Management concepts. Working knowledge of Excel, Outlook, and Word. Excellent communication, problem-solving, organizational, and time management skills. Ability to multi-task. Ability to work effectively within an interdisciplinary team. Self confident, assertive, flexible, reliable, and dependable. May be required to cover a rotating holiday or weekend on-call schedule.

One year of geriatric experience required.
