UnityPoint Health Licensed Practical Nurse (LPN) Methodist Campus M-F 8 a -4:30 pm in Peoria, Illinois

To provide professional palliative nursing care to assigned clients in support of the client’s physician and Hospice Interdisciplinary Team. Under physician orders, delivers selected treatments/procedures/tests and basic patient care for the patient on an intermittent basis while supervised by the RN.

Methodist Campus - M-F - 8 a -4:30 pm

Utilizes the nursing process to ensure quality palliative care to all clients and their caregivers/families.

· Evaluates the needs of patients and families and provides direct skilled bedside nursing care to our patients according to the plan of care under the supervision of a registered nurse.

· Evaluates patient/family needs in the preparation and execution of the interdisciplinary plan of care for each patient on the team and the effectiveness of treatments and monitors patient’s reactions.

· Performs selected acts in care of the terminally ill under the direct supervision of a registered nurse; such acts include the administration of treatments and medications.

· Informs the physician of changes in the patient condition after consulting with their supervisory RN.

· Executives on physician orders under the direction of a RN.

· Provides RN Case Manager with information for the patient’s plan of care and 60-day summary as requested.

· Assesses the patient/family response to care

· Assists the patient and family in learning appropriate self-care techniques.

· Displays compassion and patience when administering patient assessments, procedures, treatments, patient instruction and education.

Ensures quality care management by providing appropriate documentation, communication, collaboration and other follow up and by using time, supplies and resources productively.

· Collects and documents objective and subjective patient data, in accordance with the agency policies and procedures. Reports data, as appropriate, to the directing registered nurse and other members of the health care team, so that a plan of care is initiated, and changes are accurately addressed.

· Ensures communication of information with the Hospice Manager, RN Case Manager and other appropriate team members both verbally and through written reports and records.

· Attends IDT meetings

· Respects client confidentiality in all communication.

· Communicates to the physician changes in the client’s condition or status after consulting with supervising RN.

· Obtains, interprets, clarifies and implements physician orders under the supervision of RN.

· Communication and collaboration with the interdisciplinary team as client needs change

· Attends and participates in the weekly interdisciplinary team meetings and monthly nurses’ meetings.

· Participates in departmental task teams as appropriate.

· Reports to the Hospice Manager or designee all problems encountered with client care.

· Uses time, supplies and other resources productively to provide responsive and cost-effective care.

· Completes all annual education and competency requirements within the calendar year.

· Completes the IA, Death, Change of Location and Discharge logs in a timely manner.

· Submits legible clinical documentation in a timely manner using the UnityPoint Health approved abbreviate list and prohibited abbreviations.

Demonstrates punctuality and flexibility in the work schedule to meet the needs of the client and caregiver/family.

· Recognizes and prioritizes client needs to ensure quality care.

· Organizes visit schedule to ensure clients are seen within the scheduled workday.

· Utilizes time so that client care is maximized.

· Demonstrates flexibility in the work schedule to meet the needs of the client and caregiver/family.

· Provides respite care/extended hours care when assigned.

· Performs other duties as assigned by the Hospice Manager.

· Is responsible for rotating on-call coverage for weekends and holidays as assigned.

· Responds positively/willingly to request to assist in covering extra nursing visits and on-call coverage

Demonstrates evidence of professional growth and development by maintaining a current knowledge base regarding nursing and palliative care trends. Shows evidence of quality improvement in the processes of hospice care.

· Assumes responsibility for being informed about developments in health care and specifically Hospice care.

· Identifies personal and professional learning needs by participating in educational opportunities.

· Is knowledgeable and able to identify payer source requirements including, but no limited to, Medicare, Medicaid and private insurance.

· Shares education with team members, clients and caregivers/families as appropriate.

· Maintains all departmental and UnityPoint Health educational requirements including, but not limited to:

o Current Illinois licensure

o CPR certification

o Competencies

· Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.

· Completes all annual education and competency requirements within the calendar year.

· Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.


Graduate of an accredited technical school in the field of licensed practical nursing.


1-year experience as a LPN

Oncology and/or hospice training preferred.


Current licensure in good standing as a Licensed Practical Nurse (LPN) in the State of Illinois.

Must possess and maintain current Basic Life Support (BLS) certification.

Valid Driver’s License in the state of IL.

Must have Motor Vehicle Record (MVR) run annually

Must have access to own transportation. Verification of minimum auto insurance required.


Writes, reads, comprehends and speaks fluent English.

Basic computer knowledge using word processing, spreadsheet, email, and web browser.

Multicultural sensitivity.

Critical thinking skills using independent judgement in making decisions.


Use of usual and customary equipment used to perform essential functions of the position.
