The Care Manager coordinates the care and service of the member populations across the continuum of illness. This role is a Hybrid position that will allow for work in the office and from home. Geographical Areas of Focus: We are currently looking for RN Care Managers to service the Erie/Niagara region.

Key Responsibilities: * Complete a comprehensive assessment and develop a care plan for short-term & long-term needs utilizing clinical expertise. * Manages active cases based on case intensity and acuity. * Interacts continuously with member, family, physician(s), other providers and the Kalos interdisciplinary team. * Assesses the options for care including use of benefits and community resources to meet member's needs reflective in the plan of care and coordinates resources. * Acts as liaison and member advocate between the member/family, physician and facilities/agencies. * Maintains accurate records of case management activities in the electronic health record using clinical guidelines.

Required Qualifications: * Minimum of 1 year experience in MLTC required. * Current NY State RN licensure. * 5+ yrs clinical experience, preferably in home health, physician's office or public health. * While case load is focused in a particular area, you must have the ability to travel within the 6 counties of Monroe, Genesee, Erie, Niagara, Orleans & Chautauqua as needed.

Preferred Qualifications: * CCM certification preferred * 2 yrs current case management or managed care experience preferred. * Experience and knowledge with electronic health record software preferred. * Bilingual skills a plus but not required.
