Fairview Health Services RN - Home Care - Senior Services in Minneapolis, Minnesota
As a Home Care RN with the Senior Services Department of Fairview Home Care and Hospice you would be working with geriatric patients to provide skilled nursing visits in home environments. Most visits would be within Assisted Living Communities, Memory Cares, and Independent Senior Housing buildings, and occasionally to private homes. This position does require traveling to several faciities within a geographical radius and is not a position located within one facility.
The work schedule is days, Monday - Friday including a weekend (every 4th) and rotating holiday schedule.
Responsibilities include working with an interdisciplinary team consisting of an RN case manager, PT, OT, ST, SW, and facility staff to provide a comprehensive approach to their healthcare needs. This position will be providing services including foley catheter management, venipuncture, advanced wound cares, medication management, and patient/caregiver education.
Planning: Ongoing development and implementation of the plan of care
Maintains an individualized plan of care, related to the patient’s needs, in collaboration with the physician, patient and significant others.
Continually reassess the patient’s health status, environment, and support system with ongoing comprehensive assessment. Plans maximize independence of patient/caregiver.
Evaluates the patient’s progress and response to care/services delivered and adjusts the plan accordingly. Continually evaluates whether the patient’s: 1) clinical needs can be met at home 2) are able to care for themselves or have a paid or voluntary primary caregiver 3) home environment supports home care services.
Continually reassess resource utilization as patient progresses towards goals. Reviews, prioritizes and implements POC based on payer limitations.
Completes assignment by managing all anticipated patient’s care needs during visit and through next planned nurse visit.
Communicates to Case Manager and others on care team any changes in Plan of Care.
Organizing/Staffing/Directing: Coordination of services:
Communicates consistently and effectively with patient, caregiver, significant others, physicians and all team members to assure appropriate implementation and completion of the patient plan of care.
Manages patient expectations.
Advances the POC with the multidisciplinary team. Consults with the other team members as needed to make modifications in the plan. Decreases utilization of services as patient moves towards goal achievement.
Supervises paraprofessional staff in patient care.
Attends and participates in patient care rounds.
Communicates/coordinates with Clinical Resource, Utilization Review and/or MA Specialists.
Control: Maintains complete, accurate, legible and timely clinical records/ documentation on all patients according to agency policy. Completes majority of documentation at the point of care.
Meets all compliance measures (ex: MD orders, insurance authorization, patient rights). Timely submission of all documentation.
Regular communication with manager regarding concerns and problem solving.
Fiscal Accountability: 1) is a good steward of resources: discipline utilization, visits and supplies within payer limits 2) maintains productivity standards.
Professional Intervention: Provides safe, effective skilled and advanced intervention, within scope of practice according to patient need.
Provides counseling and teaching of patient, family and caregiver regarding care needs and health promotion/prevention.
Evaluates and documents response to care/services delivered.
One year recent RN experience.
Computer skills: able to document effectively in Electronic Medical Record (EMR)
Licensed as a RN and eligible to practice in Minnesota
Current CPR certification (BLS for Health Care Providers)
Bachelor’s degree in nursing
Previous RN experience in Long-term care, Assisted Living, or Transitional Care
Public Health Nurse (PHN) certification
Additional Requirements (must be obtained or completed within a period of time) :
Valid Minnesota driver’s license (or license that matches your current permanent residence)
Current and adequate automobile insurance policy
